Description
RFI - Health and Wellness Assessments **PLEASE SEE ATTACHMENT FOR COMPLETE RFI** PMO: Office of the Chief Medical Officer This Request for Information (RFI) is to gather material on requirements for developing or adopting a dynamic Health and Wellness Assessment (HWA) that is accessible to all Department of Defense (DoD) beneficiaries. The HWA should have a standardized, evidence-based, core question set that will facilitate the ability of the Military Health System (MHS) to assess the health, wellness and at-risk behaviors of its beneficiaries. Data sources include: self-reported information and biometric data from the Armed Forces Health Longitudinal Technology Application (AHLTA) and eventually the Integrated Electronic Health Record (iEHR), other possible sources (e.g. - mobile apps) and data captured from purchased care. Collated information sources will be assessed and beneficiaries will receive an online "Well-Being Plan," in addition to a medical team, which has not only been specifically designated to them, but which should also receive this plan at the point of care (POC). The Well-Being Plan consists of a personalized summary of the overall well-being of each beneficiary, and offers actionable steps that can be taken to improve well-being. Responses to this RFI should provide an understanding of existing capabilities, performance outcomes from previous experience, rough magnitude of schedule and variables which could impact schedule. Furthermore, responses should include a description of services, amenable contract structures and any unique requirements not addressed in this RFI.
