- Spine Surgeon Amendment 0002. ****NOTICE**** This notification is to inform contractors that Amendment 0002 to solicitation W81K0012R0046 was posted 2 November 2012. The Southern Regional Contracting Office has a requirement for the non-personal services of one (1) Full-time Equivalent (FTE) Orthopedic Spine Surgeon in the Department of Orthopedics at Carl R. Darnall Army Medical Center (CRDAMC), Fort Hood, TX. The base period of performance is on/about 1 February 2013 through 30 September 2013 with four twelve month option periods ending 30 September 2017. Requirement will be processed under Commercial Item procedures. This requirement is will be awarded to the offeror proposing the Best Value to the Government. The North American Industry Classification System (NAICS) code for this requirement is 621111, Offices of Physicians (Except Mental Health Specialist). Any Orthopedic Spine Surgeon that provides services under any resulting contract shall be required to possess and maintain an active, valid, and otherwise unencumbered medical license issued by the state of Texas for the length of service. The solicitation number is W81K00-12-R-0046. The solicitation is currently available at the Army Single Face to Industry (ASFI) website (https://acquisition.army.mil/asfi/solicitation_search_form.cfm) and FedBizOpps (https://www.fbo.gov/). All prospective offerors are advised that if selected for award, they must register with the Central Contractor Registration database and obtain a Commercial and Government Entity (CAGE) code prior to any award being made. Award will be made on/about 1 January 2013. Paper copies of this solicitation will not be issued, and telephone and faxed requests for the solicitation will not be honored.
