- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. MST POC: Kris Braun, Contracting Specialist, (801)432-4637 DESCRIPTION: This is a Request for Quote (RFQ) by the Utah National Guard for the items listed below. REQUIREMENTS: UnitTotal 1. 2 VI TYPHOID (2 DOSE VILES)$$ 27 PK Meningococcal (Pack of 5)$$ 76 PK Yellow Fever$$ 2. Quote must be included or quote will be considered non-responsive. 3. Include quotation number offers name and address, point of contact, phone number, e-mail address, DUNS number, CAGE number, and TIN on quote sheet. 4.Your company must be register with System for Award Management (SAM) (www.sam.gov) formally known as Central Contractor Registration (CCR). EVALUATION FACTORS: The contract type for this procurement will be a firm-fixed price and award will be made on the basis of best value to the Government, price and other factors considered. Selection will be based on the following evaluation factors, in order of importance: (1) Delivery must be by 14 Dec 2012. (2) Past performance, caliber of offeror's performance on previous contracts of a similar nature; and (3) Total cost or price. The following clauses and provisions are incorporated by reference.
