Description

The U. S. Food and Drug and Administration intend to award fixed price contract with Sprint International, Inc. for the purchase of its Federal Relay services for the period of 12 months. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subparts 12.6 and 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. Solicitation FDA-SOL-1112281-DH is issued as a Request for Quotation (RFQ) using Simplified Acquisition Procedures. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. The NAICS Code is 517110. The Contractor shall provide the following items: 0001; CLIN 401 - Voice/TTY Service; 0002; CLIN 402 - Captioned Telephone Captel (Telephonically-Based Services); 0003; CLIN 403 - Speech to Speech (Telephonically-Based Services); 0004; CLIN 404 - Video Relay Services (English and Spanish); 0005; CLIN 405 - IP Relay (Internet-Protocal (IP) Relay; and 0006; CLIN 406 - Relay Conference Captioning (RCC); and 0007; CLIN 407 - Custom Solutions. All contractor and subcontractor employees are required to sign the Contractor's Commitment to Protect Non-Public Information (NPI) Agreement (Form FDA 3398) which will be provide at time of award. FAR 52-217-8 Option to Extend Services (NOV 1999), FAR 52-217-9 Option to Extend the Term of the Contract (MAR 2000) FAR 52.216-18 Ordering (OCT 1995) FAR 52.216-19 Order Limitations (October 1995) FAR 52.216-22 Indefinite Quantity (OCT 1995) 52.227-14, Rights in Data-General (June 1987), Alternate I (June 1987), Alternate II (June 1987), and Alternate II (June 1987), 52.217-7 Option for Increased Quantity- Separately Priced Line Item. (MAR 1989), 52.232-18 Availability of Funds (Apr 1984) are hereby incorporated by reference and apply to this acquisition. The Government intends to make one award. All responsible sources that can provide and meet the above requirement shall submit a written quotation. Quotations must be received by November 30, 2012 10 a.m. Eastern Time.
