- Performance Work Statement 1Contractor shall maintain equipment and software in good repair and working order. A full parts and labor warranty is required. Maintenance Contract for 24/7/365 TAC and 2 hour response time for emergency situations for VAMC telephones (PCX 4400). 2Software Evolution Services (SES) shall be included in all proposals and which shall include: -24x7x365 remote technical assistance available in 4 languages -Software patches and maintenance releases through the term of the contract -Problem Diagnosis services -Immediate access to all major product enhancements and minor software releases -A migration from 8.0 to 9.0 software Revision (or the most up-to-date version at the time the contract) is accomplished within 30 days contract award. 3. Contractor shall be on site within a 2-hour (24 hours per day) response time for all emergency maintenance calls; for non-critical service, contractor shall respond within 4 hours; for non-service affecting service, contractor shall provide service within 24 hours. These call back service shall be provided as requested 24 hours per day, 365 days per year including Federal holidays at no additional charge to the Government. 4. The contractor shall supply a full parts and labor warranty as detailed on any new equipment, replacement parts installed by the contractor or subcontractor. Contractor shall maintain the Software and Equipment at Cheyenne VAMC, Fort Collins CBOC and Greeley CBOC, in accordance with OEM standards, and standard commercial practices. Warranty to include all existing and new equipment (including VoIP & Digital phones, PCX cards & Gordon Kapes unit) the Cheyenne VAMC Surplus equipment. (Exclusions: fire, acts of God or customer abuse). 5. Contractor shall provide, at each location, a qualified primary technician assigned from the local office to service and maintain the VA owned OmniPCX 4440 telephone system. The local service office shall also provide a backup technician in the event of vacation, sickness, conflicts, etc. of the primary technician. Instructions for obtaining maintenance shall be provided by vendor. The contractor shall provide to the Telcom manager or designee a list of names and qualifications for each Technician. 6. Upon completion of the services, the Contractor shall report to the Contracting Officers Representative (COR) or designee and a written report of services performed shall be furnished to the COR or designee. The service reports shall advise of any repairs or repair parts not required to be furnished under the contract that is necessary to maintain the equipment in perfect operating condition. For other than normal hours of service the contractor shall forward a copy of the service report to the COR via fax (a fax number will be provided after award). 7. The Contractor shall maintain a complete orderly and chronological file including drawings, complete part lists and copies of all reports as required by these specifications. A record of all callbacks and repairs shall be kept by the Contractor indicating any difficulty experienced and the corrective measures taken to eliminate these difficulties. This file shall be available for inspection upon request, and a copy of this complete file shall be furnished to the VA Medical Center during the last month of the purchase order period. 8. Work required in the performance of the contract shall be performed during regular administrative working hours, 8:00a.m to 4:30 p.m., Monday through Friday, including Federal holidays. Upon arriving at the VA to perform services, the Contractor shall report to the COR or his/her designee. 9. For a call initiated by a system alarm, the alarm handling system automatically transfers the request to the service queue. If the problem can be cleared remotely, the contract shall perform the clearing action, and closes out the problem in their service delivery support system. IF the service request was initiated by a customer call, the problem is also closed out with the customer. The request is traced by the contractor and they shall be responsible for achieving closure with the contractor customer. 10.On-site service activity shall be initiated by a transfer of the Trouble Ticket from TAC/Dispatch support system to the appropriate technician for each customer system covered by this maintenance service Agreement. The contractor shall contact the customer and inform the customer of estimate on-site arrival time. 11.Upon completion of the service call the contractor shall inform the customer, and close the service ticket in the service delivery support system. If the contractor is unable to resolve on the initial on-site visit, the contractor shall be responsible for informing the customer of the current status, and the plan for correct action. The plan of corrective action shall also include an estimate period for which the repair shall be completed. 12.If an assigned technician cannot provide service delivery in an appropriate timeframe, the contractor shall arrange for an escalation of the problem to the next higher level of TAC and inform the "escalation" manger who shall inform the customer what is being done and an estimated time of resolution. 13.Maintenance price includes active and inactive ports. 14.Maintenance price includes coverage of voicemail equipment. 15.Maintenance price includes 10% off hardware refresh items plus 5% credit on any purchase of hardware. 16.Maintenance price includes Alcatel Software revisions (bug-fix) to fix performance issues. 17.Maintenance Price includes annual checkup of switch, occurs twice per contract year. 18.Maintenance price includes telephone replacement of same type. 19.Maintenance price includes one time, repository site for backup of database, site topology, circuit ID, carrier contact. 20.Maintenance price includes monthly backup to Dicellc ftp server. Troubles 1. Vendor shall provide Cheyenne VAMC with a single point of contact for all reporting of trouble on covered services. Troubles shall be documented; analyzed and appropriate resources shall be applied to correct the problem. There will be three levels of response. ? Non-Service Affecting - shall be provided within 24 hours after receipt of notice from the COR or designee. Initial VA contact may be telephone, FAX or email to the contractor. During weekends or holidays, the contractor shall provide service on the next regular business day. ? Service Affection - non -critical stations and service shall be responded to within 4 hours and shall be repaired according to VA customer's requirements. ? EMERGENCY SERVICE - Emergency service shall be furnished within 2 hours after receipt of notice. 2. For other than normal working hours, the contractor shall respond to all calls as stated above and dispatch a service technician accordingly. A technician/system engineer shall have the capability to access systems remotely from home or on the road and shall be available at all times to assist in restoration of service. Routine Maintenance 1. Contractor shall certify that a repairman shall be on premise and initiate troubleshooting and repair within 24 hours after receipt of a routine maintenance call. After receipt of an emergency maintenance call the contractor shall certify that a repairman shall be on the premise within two (2) hours (24 hours per day) to initiate trouble shooting and repair service. The Cheyenne VAMC Director or their representative, to include the Telecommunications Manager, will determine whether service requirement is an emergency or routine service call. Spare Parts 1. A minimum spare parts list as required by the OEM for proper maintenance of the total installed system has been purchased by the Cheyenne VAMC at the time of installation and is maintained in the telephone switch room. Contractor shall replace these spare parts as used by the contractor to restore defective equipment during the warranty period.
