- REVISED 12 SCHEDULE B REVISED 11 COMPENSATION PLAN PRICING W91YTZ-12-R-0039-0006 CONFORMED VERSION W91YTZ-12-R-0039-0006 MOD VERSION W91YTZ-12-R-0039-0006 is being added. This Amendment must be signed and included into your revised proposal. The Period of Performance has changed. Therefore, the response date is being extended and proposals must be resubmitted. The Contracting Office apologizes, but the Past Performance Questionairres must be resubmitted. The Contracting Specialist had technical difficulties and when the computer was redacted, the Past Performance Questionairres were inadvertently deleted. Several vendors have sent their proposals to the wrong location. It is imperative that the proposals are sent to the address located in Addendum 52.212-1. PLEASE READ THE AMENDMENT CLOSELY PLEASE REVISIT ADDENDUMS 52.212-1 AND 52.212-2.
