Description
The attachment is a complete conform copy of RFQ #N00183-13-T-0088. This RFQ is unrestricted & will close on Monday, 23 December 2012. Please adhere to all information & instructions listed in this RFQ before submitting a quote. Also, quotes can be submitted to email address sherita.martin@med.navy.mil. This solicitation is unrestricted. Requirement to be awarded on a Brand Name, All or None basis. Technical evaluation of vendor's quote will consist of technical capability to provide and perform product specifications as stated in Section B & price. Items on this RFQ are for drop shipment (standing order) of Simplex P Bone Cement w/Tobramycin manufactured by Stryker Orthopaedics for the Main OR at Naval Medical Center-Portsmouth, VA. Quotes should reflect pricing on all items for consideration. Please provide government & destination pricing. Also, state warranty information for each CLIN number (Line Item). If freight charges apply, indicate if charges will be "per shipment" or "single one-time charge". Contractor shall supply a current published price list or if material is on a FSS contract, a GSA/VA Federal Supply Schedule contract for supplies &/or services being procured. Refrigerated, frozen, or hazardous material must be indicated as such & a copy of the MSDS (Material Safety Data Sheet) must be supplied for any hazardous material. Specify any special handling instructions. Solicitation closes on Monday, 24 December 2012.
