Description
Market Research Questionnaire THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY - Solicitations are not available at this time. This notice does not constitute a commitment by the Government. The Bureau of Prisons has a future requirement for Orthopedic Surgeon services for inmates at the Federal Correctional Institution located in Ray Brook, NY. The North American Industry Classification System (NAICS) Code is 621111 (Size Standard: $10.0 million). All duties shall be performed in accordance with standards and methods generally accepted within the field of Orthopedic services, and compliance with all Federal and State policy and regulations. The contractor shall perform history and physical examinations to obtain a diagnosis in preservation, restoration, and development of function of the extremities, spine and associated structures. This may be done by medical, surgical and physical methods. The contractor must be knowledgeable in performing arthroscopy and arthroscopic surgery. The contractor may perform intra-articular injections into joints. The contractor may interpret x-ray films to obtain a diagnosis and advise a surgical procedure to correct an abnormality. The contractor shall be experienced as an Orthopedic Surgeon and have a current and valid license to practice medicine in the state of New York. It is estimated that the contractor will provide 48 sessions (session is defined as one hour) per year. The service will normally be performed in one four hour clinic each month between the hours of 7:30 a.m. to 4:00 p.m., Monday through Friday. The Government contemplates awarding of an indefinite-delivery, requirements-type contracts with firm-fixed unit prices resulting from the future solicitation. The contract period shall consist of a 12 month base period with four 12 month option periods for renewal. A Government solicitation document to provide these services is being developed, and a contract may or may not result. Market research is being conducted to determine interest and capability of potential sources for this requirement. All information submitted in response to this announcement is voluntary. The Government will not pay for information requested nor will it compensate any respondent for any cost incurred in developing information provided to the Government. The Market Survey Questionnaire will be available on or about December 31, 2012 and will be distributed solely through General Service Administration's Federal Business Opportunities web site http://www.fedbizopps.gov/. The site provides downloading instructions. All future information about this acquisition, including solicitation and/or amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information regarding this acquisition. Faith-Based and Community-Based Organizations can submit offers/quotes equally with other organizations for contracts for which they are eligible. The response date for the market survey is January 10, 2013. No collect calls will be accepted. No telephone request or written requests for the market survey will be accepted. All responses to the Market Survey Questionnaire may be submitted in hard copy to U.S. Department of Justice, Federal Bureau of Prisons, Field Acquisition Office, U. S. Armed Forces Reserve Complex, 346 Marine Forces Drive, Grand Prairie, TX 75051, Attn: Catherine Corcoran, Contracting Officer. The questionnaire may also be emailed to ccorcoran@bop.gov.
