- The purpose for amendment is to extended the closing date of this solicitation from December 27, 2012 to January 10, 2013 at 2:00 p.m. EST. The Federal Bureau of Prisons, Transitional Drug Abuse Treatment Contracting; Washington, D.C. is seeking quotes from sources having the ability to provide community-based outpatient Substance Abuse and Mental Health Services in Sioux City, Iowa. Location of services should be within a 10 miles radius of the U.S. Post Office which is located at 214 Jackson Street, Sioux City, Iowa 51101. Performance periods under this contract will be for a one-year base period to begin on October 1, 2012 with four (4) one-year option renewal periods with service ending (if all options are renewed) on September 30, 2017. The services required for this contract include a Base Year, Option Year One, Option Year Two, Option Year Three and Option Year Four, respectively. The Following services are required and may be sub-contracted to another licensed contractor. The requirements for Transitional Drug Abuse Treatment and Mental Health Services for male and female Federal offenders includes: Base Year: Estimated number of Inmates = 20 Substance Abuse Treatment Services: Intake Assessments & Report (one per authorized inmate) for 16 inmates = 16 assessments Individual Counseling Sessions (30 minutes = 1 unit) 48 units (per inmate) x 16 = 768 units Group Counseling Sessions (30 minutes = 1 unit) 72 units (per inmate) x 16 = 1,152 units Family Counseling (30 minutes = 1 unit) 8 units (per inmate) x 2 inmates = 16 units Transportation = (1 unit per inmate) x 20 inmates = 20 units Mental Health Treatment Services: Mental Health Intake Assessment & Report (one per inmate) for 4 inmates = 4 units Mental Health Individual Counseling (30 minutes =1 April 2011- No Masters. The request for quotation will be available on or after August 1, 2012 and will be distributed solely through the General Services Administration's Federal Business Opportunities web site http: //www.fedbizopps.gov. Hard copies of the quotation will not be available. The site provides downloading instructions. All future information about this acquisition, including quotation amendments, will be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. The estimated closing date is on or about August 13, 2012. Faith-Based and Community -Based Organizations can submit offers/bids/quotations equally with other organizations for contracts for which they are eligible. This requirement is unrestricted. The BOP anticipates awarding a firm-fixed price contract. All responsible business concerns may submit a quote which will be considered by this agency.
