ATTACHMENT 1 PERFORMANCE WORK STATEMENT Facilities Configuration Management Program (FCMP) Solicitation ATTACHMENT 4 QASP ATTACHMENT 3 RESPONSIBILITY MATRIX ATTACHMENT 6 HISTORICAL WORK REQUESTS ATTACHMENT 9 CONTRACTOR PERFORMANCE SURVEY ATTACHMENT 11 PRICING SUBMISSION SHEET ATTACHMENT 7 PAST PERFORMANCE DATA ATTACHMENT 8 PAST PERFORMANCE QUESTIONNAIRE ATTACHMENT 5 WAGE DETERMINATION ATTACHMENT 2 CONTRACT POSITION DESCRIPTION MATRIX ATTACHMENT 10 DD 254 Facilities Configuration Management Program (FCMP) Support Services Requirement Washington Headquarter Services (WHS) Acquisition Directorate (AD) is issuing a combined synopsis/solicitation (HQ0034-13-R-0003) in order to locate a qualified contractor, which can provide Facilities Configuration Management Program Support Services for the Pentagon and its surrounding facilities. The Facilities Services Directorate (FSD) maintains and operates Department of Defense (DoD) facilities in the National Capitol Region (NCR-Northern Virginia, DC, and MD) and the "Site" facility, located in Adams County, PA (Refer to PWS for definition of the "Site"). This RESTRICTED ACQUISITION is set aside - 8(a) competitive for SBA Region III. In order to qualify for this program, an offeror must be a certified 8(a), within Region III, prior to the closing date of the solicitation. All other firms are deemed ineligible to submit offers. The North American Industry Classification (NAICS) code for this requirement is 541513 (Computer Facilities Management Services). The solicitation will be awarded as a single award - firm fixed price indefinite delivery/indefinite quantity type contract. The geographic locations where the work will be performed will be the Pentagon and its surrounding areas as specified in the Performance Work Statement (PWS). Offerors shall refer to the Facilities Configuration Management Program Support Services solicitation and its attachments, for detailed information regarding this requirement. Questions regarding the solicitation must be received by 02:00pm on 14 January 2013. Proposals must be received by 10:00am on 31 January 2013. All contractors shall be ORCA registered http://orca.bpn.gov now (www.SAM.gov). Contractors; in accordance with FAR 52.204-7, Central Contractor Registration; contractors must be registered in the Central Contractor Registration (CCR) database now (www.SAM.gov) before a contract can be awarded to your company. Registration may be accomplished at (www.SAM.gov). The solicitation will be distributed solely through the Federal Business Opportunities web-site http://www.fedbizopps.gov/. Hard copies of the solicitation document will not be available. The site provides downloading instructions. All future information about this acquisition, including solicitation amendments, will also be distributed solely through this site. Interested parties are responsible for monitoring this site to ensure that they have the most up-to-date information about this acquisition. You must register on the FBO site in order to receive notification of changes to the solicitation. When the solicitation is downloaded, it is important to read the solicitation COMPLETELY, as there are milestones that MUST be met by the prospective offeror in order for the proposal to be considered responsive.
