ATCH: 3 DFARS 252.209-7999 Tax Liability ATCH 2: Pricing Sheet ATCH 1: PWS for Tobacco Cessation 1. 21st Contracting Squadron, Peterson AFB, CO hereby issues this combined synopsis/solicitation for commercial items prepared in accordance with (IAW) FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; written quotes are being requested and a written solicitation will not be issued. 2. This Request for Quotation (RFQ) is issued as solicitation, FA2517-13-T-6009. Only written quotes will be accepted in response to this RFQ (no oral offers will be accepted). All firms or individuals responding must be registered with the System for Award Management (SAM). 3. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-62 and through Department of Defense Acquisition Regulation Change Notice 20121212. It is the contractor's responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at http://farsite.hill.af.mil/vffar1.htm. 4. This acquisition is procured as full and open competition. The NAICS code for this requirement is 813212, Voluntary Health Organizations. The size standard is $25.5M and a single award will be made as a result of evaluation of quotes. 5. The requirement is for Tobacco Cessation service to be accomplished through a phone hotline for Schriever AFB, CO. The contract period of performance is one base year and four option periods. Estimated base year start date is 11 February 2013. Use pricing worksheet to propose firm fixed price for the requirement. This commercial service is being acquired in accordance with the attached Performance Work Statement (PWS) (ATCH 1). 6. Commercial services shall be priced as FOB destination to Peterson AFB, CO. Unless otherwise specified in the contract, the supplier will be responsible for the performance of all inspection requirements and Quality Control (QC). 7. The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items applies to this acquisition. All companies must be registered in Wide Area Workflow (WAWF) at https://wawf.eb.mil at time of quote submittal. All companies must be registered in the System for Award Management at www.sam.gov in order to be considered for award. The Government will not provide contract financing for this acquisition. The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation which will be most advantageous to the Government, price and other factors considered. 8. The provision at FAR 52.212-2 Evaluation--Commercial Items applies to this acquisition. a. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Basis of award is the best value, Lowest Price Technically Acceptable (LPTA) using the following evaluation factors: (1) Technical Capability Approach (2) Staffing Plan (3) Price Technical approach and staffing, when combined, are more important than price. b. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). c. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. 9. The provision at FAR 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this acquisition. All vendors must be registered in Online Representations and certifications (ORCA) (https://orca.bpn.gov)) at the time of Quote submittal. 10. The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to the acquisition. 11. The clause at FAR 52.212-5 Contract Terms and Conditions required to Implement Statutes or Executive Orders - Commercial Items (DEVIATION), applies to this acquisition. 12. The following FAR provisions and clauses are applicable to this solicitation: a. FAR 52.217-8 Option to Extend Services b. FAR 52.217-9 Option to Extend the Term of the Contract c. FAR 52.223-18 Encouraging Contractor Policies to Ban Text Messaging While Driving d. FAR 52.225-13 Restrictions on Certain Foreign Purchases. e. FAR 52.225-25 Prohibition on Contracting with Entities Engaging in Sanctioned Activities Relating to Iran-Representation and Certification. f. FAR 52.233-4 Applicable Law for Breach of Contract Claim g. FAR 52.222-41 Service Contract Act of 1965 h. FAR 52.222-42 Statement of Equivalent Rates for Federal Hires 13. The following DFARS provisions and clauses are applicable to this solicitation: a. DFARS 252.212-7001, Contract Terms and Conditions Required to Implement Statues or Executive Orders. The following applies: (1) Clause Para (a) (2) Clause Para (b) only the following clauses apply: DFARS 252.203-7000 DFARS 252.232.7003 (3) Clause Para (c) applies in its entirety DFARS 252.232-7006 14. The following AFFARS clause is applicable to this solicitation: a. AFFARS 5352.201-9101 Ombudsman 15. The Defense Priorities and Allocation Systems (DPAS) does not apply to this request for quote.. 16. Quote submission: Quote shall include price information (ATCH 2); quote expiration date, tax liability information (ATCH 3), DUNS, Cage Code, tax ID, technical approach, staffing plan, and terms and conditions.
