Sources Sought This is a Sources Sought Announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS code 621111 Office of Physicians (except Mental Health Specialists) Size Standard $10 Million. The Network Contracting Office at the Canandaigua VA Medical Center (VISN 2) has a requirement for a contractor to provide ENT/Otolaryngologist services for beneficiaries of the Stratton VA Medical Center, Albany, NY. All work performed under this contract will be performed at the Stratton VA Medical Center. Board Certified/Board Eligible general Otolaryngologist(s) with sub-specialty training or significant experience in Otology are needed for on-call and emergency consultation, surgery, and follow-up services. Contractors must be able to respond to on-call within 15 minutes of initial call by phone or within 60 minutes of initial call in-person if on-site clinical management is required. Responses to this Sources Sought must include the following information: 1. General information about your company: name, address, contact person name, phone number, fax number, email address(s), DUNS number, and company website if available. 2. Type of business, such as: 8(a), Small Business, HubZone Small Business, Small Disadvantaged Business, Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. Responses must be received no later than 3:00 p.m. Wednesday, January 16, 2013. No solicitation document is available at this time; this notice is to acquire information only.
