Description

Network Contract Office 11 (NCO 11) will be soliciting proposals for artificial limbs for veteran beneficiaries of Veterans Integrated Service Network 11 (VISN 11). Contractor shall fabricate artificial limbs in strict conformance to the prosthetic prescription that has been provided by the VA Amputee Clinic. Contractor shall have a full-time American Board Certified (ABC) or Board for Orthotist-Prosthetist Certification (BOC) on staff and the contractor's facilities shall be approved by the Facility Accreditation Program of either the ABC or BOC. Request for Proposal VA251-13-R-0001 will be issued on or around February 12, 2013. Anticipated award date is May 30, 2013 for a period of one year with four one-year option periods to be exercised at the government's discretion. This is a multiple award acquisition with multiple awards for each of the VISN Amputee Clinics located in Ann Arbor, Battle Creek, Detroit and Saginaw, MI; Danville, IL; and Indianapolis and Marion, IN. No telephone requests will be honored. The government is not responsible to make available paper copies. Questions must be submitted in writing to the contracting officer. Verbal questions will not be addressed. All responses to questions which may affect offers will be incorporated into a written amendment to the solicitation. Offeror's are encouraged and are responsible for routinely checking the web site for any solicitation amendments that may be issued throughout the bid period. No individual notification of amendments will be provided. Proposal due date and time will be included in the solicitation document. This is a multiple award acquisition with a partial set-aside for Small Business. The NAICS code for this requirement is 339113. The size standard is 500 employees.
