Description
This is not a solicitation announcement. This is a sources sought notice for the identification of potential small business vendors who have the capacity to provide total parenteral nutrition (TPN) intravenous feeding method indicated for those patients who require complete bowel rest or fail to obtain nourishment from a dysfunctional gastrointestinal tract. The full daily caloric requirement can be met with parenteral delivery of macronutrients (carbohydrates, amino acids, and fats), as well as the replacement of micronutrients (electrolytes, minerals, vitamins, and other additives), tailored to the individual patient. The majority of calories are supplied with carbohydrates, followed by lipids, then amino acids. TPN solutions at are prepared by a technician and a pharmacist using aseptic technique in an ISO Class 7 sterile room in an ISO Class 5 Laminar Flow Hood to ensure sterility. The pharmacy staff uses a Baxter compounding machine to custom prepare the macronutrient base of the TPNs. Then the electrolytes, minerals, and other additives are individually and manually drawn up into syringes and injected into the TPN base. There are several series of double and triple checks throughout this process to ensure the complex TPN solution is formulated correctly. Currently, Bay Pines dictates a standard hang time of 1800 for all TPN solution bags; to meet this hang time, all orders should be submitted by 1300. Preparation and delivery of TPN must meet VHA, FDA and JCAHO admixture standards. A total of approximately seven (7) TPN will be delivered daily, three hundred and sixty-five (365) days a year to 10,000 Bay Pines Blvd., Bay Pines, FL 33744. See attached Statement of Work following this announcement. The North American Industry Classification System code (NAICS) is 325412 and the size standard is 750. All interested and capable respondents are requested to provide the following requested information. All responses to this notice shall include: company name, address, point of contact, phone number, DUNS number (www.dnb.com), confirmation they can provide and service the equipment, supplies, and to perform and report the required aquatic services of fountains and waterways. Telephone responses will not be accepted. Reponses should be sent via e-mail to Janice.fornaro@va.gov not later than 5:00 PM EST, January 22, 2013. All contractors interested in doing business with the Government must be registered in the System for Award Management (SAM) database. Service-Disabled Veteran-Owned Small Businesses and Veteran-Owned Small Businesses should also indicate whether the company is verified by the VA in the Vendor Information Pages (VIP) Database at www.vetbiz.gov. SOURCES SOUGHT FOR PATIENT SPECIFIC TOTAL PARENTERAL NUTRITION (TPN) ADMIXTURE The Department of Veterans Affairs, Bay Pines VA Healthcare System (VAHS) intends to award a contract to a responsible provider of patient specific Total Parenteral Nutrition (TPN) admixture who can provide TPN services to the Bay Pines VAHS pharmacy in a timely manner as indicated. Contractor/supplier shall abide by United States Pharmacopeia Chapter 797 concerning: (a) Personnel training in aseptic techniques (b) Verification of compounding accuracy and sterility (c)Verification of Automated Compounding Devices (ACDs) for Parenteral Nutrition Compounding (d)Environmental quality and control including ISO class of 5 or better (e) All other requirements and recommendations outlined Areas subject to monitoring may include any or all of the following: (a)Ingredients and Devices (b) Equipment (c) Accuracy (d) Precision (e) Inspection of solution dosage forms and review of compounding procedures (f) Compounding Accuracy Checks (g) Sterility Testing (h) Identity and strength verification of ingredients 1.Contractor shall provide delivery of the admixture of TPN solution to the I.V. Pharmacy daily, Sunday-Saturday, seven (7) days per week and 365 days per year to include all holidays, at a time acceptable to the contractor and Pharmacy Service before 7:00 p.m. Federal Holidays observed by the government are as follows: New Year's Day Martin Luther King Day Presidents' Day Memorial Day Fourth of July Labor Day Columbus Day Veterans' Day Thanksgiving Day Christmas Day Any other day specifically designated as a Federal Holiday by the President of the United States. Holidays that fall on a Sunday are observed the following Monday; Holidays that fall on a Saturday are observed the preceding Friday. 2.The Bay Pines VA Health Care System shall send via facsimile, personal computer and/or contractor proprietary software, or direct computer interface the appropriate orders necessary for the contractor to accurately admix TPN. No Protected Health Information/Electronic Protected Health Information (PHI/EPHI) shall be transmitted. The orders for the daily TPN solutions will be sent to the contractor before 12:00 noon EST. All orders discontinued prior to the requested delivery time shall not be charged as they will be used for quality assurance processes by the contractor. 3.Contractor shall perform all quality control procedures as required by Federal, State, local laws, regulations and governmental orders, including applicable Board of Pharmacy regulations and USP Chapter 797, in providing items or service pursuant to this agreement. Contractor shall provide the government with a copy of their quality control plan upon contract award and at the request of the CO/COTR. 4.Contractor shall comply with all applicable standards established by the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) or other accrediting body whose standards meet or exceed those established by JCAHO. The contractor staff, affiliate personnel, and any other providing services shall be properly trained and credentialed and properly reviewed according to the requirements set forth by JCAHO or other accrediting body whose standards meet or exceed those established by JCAHO. 5.The contractor shall guarantee a monthly service level of 100% of all orders received prior to the cut off time as mutually agreed in writing by the Contractor and Bay Pines VAHCS Pharmacy Service. The contractor shall fulfill all change orders received after the cut off time but prior to physical shipment. 6.Delivery Schedule: Delivery specified by the Government on FOB Destination within consignee's premises contracts are the dates material is due in, not the shipping dates. Delivery shall be made for the times indicated in this contract to the IV Pharmacy. Contractor shall provide routine delivery and pick-up as many times as required. The contractor shall be required, as deemed necessary by the Bay Pines VA Healthcare System, to furnish material before 7:00pm. The period of performance for this contract will be for one (1) base year from date of award with four (4) one (1) year options to extend the term of the contract based upon the requirements of the government as stated herein. The estimated value of this procurement is between $150,000.00 and $500,000.00. The applicable North American Industry Class System Code may be 325412 Pharmaceutical Preparation Manufacturing with a small business size standard of 750 employees. At the present time this acquisition is expected to be issued as a sole source as according to market research there have not been any other companies capable of providing patient specific TPN admixture services who can provide delivery in a timely manner to the Bay Pines VA Medical Center as indicated in this sources sought.
