- GFM Ultrasonic Cutter I/H Surveys GFM Ultrasonic Cutter Maintenance CDRLs GFM Ultrasonic Cutter Maintenance PWS GFM Ultrasonic Sutter Maintenance Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Request for Proposal (RFP) number is FA8224-12-R-0087. The Government contemplates award of one (1) contract resulting from this solicitation. This RFP is not restricted. This acquisition is for the procurement of planned annual maintenance, unscheduled repairs, telephone and email technical support and parts replacement for the GFM Ultrasonic Cutter within the 309th Commodities Maintenance Group (CMXG), Hill Air Force Base, UT. See attached solicitation, Performance-Based Work Statement (PWS) dated 15 January 2013, CDRLs, and Industrial Hygiene (1/H) Surveys for full details of contract requirements. NOTE: Government provided I/H Surveys are for informational purposes only and cannot be used by the contractor for his own occupational health compliance purposes. Contractors will be required to perform/validate IH surveys as necessary and are advised to propose accordingly. Anticipated period of performance is one (1) base year with two (2) option years. Offerors shall provide FFP pricing for CLIN 0001, 0101 and 0201 (Maintenance, Calibration & Tech Support CLIN). CLIN 0002, 0102 and 0202 (Unscheduled Repair and Replacement Parts CLIN) Offerors shall submit fully loaded labor rates and labor categories for services required under this CLIN) Note: Government will set the ceiling for these CLINs and pricing provided will not be used to calculate Offerors Total Evaluated Price (TEP). CLIN 0003, 0103 and 0203 (Travel) Offerors shall not provide pricing for this CLIN, the Government will set the ceiling for these CLINs. CLIN 0004, 0104 and 0204 (Data) are CDRLs that are not separately priced. ANTICIPATED AWARD DATE: The contract is expected to be awarded on or about, 1 March 2013. Offerors are advised to review 52.212-1 Instruction to Offerors and 52.212-2 Evaluation, clauses in the solicitation for instructions to Offerors and evaluation criteria.
