Description

Solicitation Number: VA-241-13-R-0116 Notice Type: Combined Synopsis/Solicitation Synopsis: The VA Boston Healthcare System intends to negotiate and procure the requirement for Radiopharmaceuticals/isotopes. A firm fixed price contract with a base and (2) option years is anticipated and shall be awarded. This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in subpart 12.6, in conjunction with the policies and procedures for solicitation, evaluation, and award as prescribed under FAR 13.1, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number is VA-241-13-R-0116 and is issued as a Request For Proposal (RFP) (iii) The provisions and clauses incorporated into this solicitation document are those in effect through Federal Acquisition Circular 2005-62, 20 DEC 2012. Provisions and clauses incorporated by reference have the same force and effect as if they were given in full text. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at https://www.acquisition.gov/far/ (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR) (iv) This solicitation is issued as full and open competition. The North American Industry Classification System (NAICS) code is 325412, and the size standard is 500 employees. (v) This requirement consists of: (1) Delivery of isotopes for Nuclear imaging daily. (2)Hours of Coverage: Daily (3)Response Time: N/A (4)Qualification of Vendor: Vendor shall have the ability to deliver Radiopharmaceuticals to VAMC White River Junction, VT. (5)Specifications of Work: Orders for isotopes will be placed daily prior to the next business day. Isotopes will be delivered to the VAMC White River Junction facility the currier will be met at the facility and escorted to the Nuclear Medicine department by VA police during off tours. The isotopes will be placed in the storage locker until authorized VA personnel are available place product into the hot lab. (6)Guaranteed Uptime: N/A (7)Updates: All updates of information will be provided to Chief Radiologic Technologist and Nuclear Medicine department of the VAMC White River Junction. (8)Documentation Requirements: All documentation shall be provided to Chief Radiologic Technologist. (9)Services and parts not covered by contract: None (10)COR: Preston Robison (vi) The following STATEMENT OF WORK is a detailed description of the requirements for this solicitation: 1.PURPOSE. The Department of Veterans Affairs is requiring orders for isotopes that will be placed daily prior to the next business day. Isotopes will be delivered to the VAMC White River Junction facility the currier will be met at the facility and escorted to the Nuclear Medicine department by VA police during off tours. 2.OBJECTIVE. The contractor shall place the isotopes in the storage locker until authorized VA personnel are available place product into the hot lab. Proposal shall also include options to extend the term of the contract for periods of up to 6 months not exceeding 2 option eriods. PERIOD OF PERFORMANCE. The ANTICIPATED period of performance shall be from the date of award for a 9 month period with the option to extend services for up to 6 month periods. PLACE OF PERFORMANCE. White River Junction VA, 215 North Main Street, White River Junction, VT 05009 Type of Contract. A Firm- Fixed Price Contract will be issued for this effort. PRICING. Vendors should also submit pricing to reflect the daily rate for radiopharmaceuticals/isotopes and delivery.
