- This notice is amended to add the following: The CDC will accept the lowest cost technically acceptable proposals. The Centers for Disease Control and Prevention (CDC) intends to issue a sole source firm fixed priced purchase order to PerkinElmer, 710 Bridgeport Avene, Shelton, CT 06484-4794. The purchase order is for total maintenance and repair for the following instruments manufactured by Perkin Elmer or its subsidiaries: JANUS Platform Liquid Handling System (Serial #DGO8061376); Victor 2 Plate Reader (Serial #4205378); and Chemagen Nucleic Acid Prep Station (Serial #MSM1). The Period of Performance for the base year is on or about March 1st, 2013 to February 28th, 2014. Additionally, four (4) option years are being sought. Full Service coverage which covers unlimited on-site priority repair service. Requests for services should be responded to within 2 work days. The instrument should be returned to Original Equipment Manufacturer (OEM) and testing standards no later than five days post-notification. One (1) scheduled planned preventive maintenance (PM) visit per contract year. During the PM, the instrument should be cleaned, calibrated, and tested to ensure it meets or exceeds OEM standards of operability. The vendor is responsible for all costs associated (parts, labor, shipping and travel) for all service calls (PM and unscheduled repairs). After each PM and repair, the engineer needs to provide a service report of what was done to the instrument on company's template. The certificate contains, at a minimum, the specifications of the parameters tested and all corrective actions taken. Repairs should be carried out on-site. If an instrument needs to be taken off-site the vendor needs to provide an equivalent or better replacement at no additional cost to the government. Parts must be new OEM. (Used/non-OEM parts might be considered, but may only be utilized with prior permission of the contracting officer representative (COR) and this permission must be obtained every time used/non-OEM parts are considered.) Services are performed during normal business hours 8 am to 4 pm, Monday-Friday. Computers and software associated with the operation of any and all instrumentation must also be fully covered for operability. The CDC believes that this requirement can only be met by one provider. Note 22 applies herein. The statutory authority permitting other-than-full-and-open competition is 41 U.S.C., 253(c) (1): FAR 6.302-1. Only one responsible source and no other services will satisfy agency requirements. Interested persons may identify their interest and capability to respond to the requirement. This notice is not a request for competitive proposals. A determination by the Government not to compete this proposed contract will be based on responses to this notice and is solely within the discretion of the Government. Information received will be considered solely for informational purposes. This procurement is set-aside for small business. The proposed contract action is for commercial items which the Government intends to solicit and negotiate with only one source under authority of FAR 13.106-1(b). Interested persons may identify their interest and capability to respond to the requirement. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION; QUOTES ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. The following provisions apply to this acquisition; 52.212-1 Instructions to Offerors - Commercials Items (FEB 2012) applies to this acquisition; 52.212-2, Evaluation - Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed system to meet the requirements stated herein, and (2) price. The clause 52.212-4, Contract Terms & Conditions - Commercial Items (FEB 2012) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination; The clause at 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (JUL 2012) applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (7), (9), (11), (12), (14), (15), (17), (18), (20) and (24). Quotes will be due January 29, 2013, by 1:00 PM EST at the Centers for Disease Control and Prevention; Acquisition and Assistance Branch B; Attn: Tonya S. Justice, 2920 Brandywine Road, Atlanta, GA 30341 or via email to tjustice@cdc.gov. All responsible sources that can meet the above requirements may submit a quote, which will be considered by the Agency. All vendors need to be registered in the Central Contracting Registry (CCR) prior to award of a Federal contract. The preferred method for completing registration is via the World Wide Web at http://www.sam.gov. A handbook and other information are available at the website, including information on how to obtain a DUNS number, which is required to begin the registration process.
