Combined Synopsis/Solicitation in PDF format PWS - Performance Work Statement - Tricare Health Benefits Advisor This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ). This document and incorporated provisions and clauses are those in effect for Federal Acquisition Circular 2005-62. This acquisition is unrestricted. //Schedule of items: This requirement is for Tricare Health Benefits Advisor //CLIN 0001: 12 months Services to be performed in accordance with Performance Work Statement Period of Performance: 1 April 2013 - 31 March 2014 Place of Performance: 426th Air Base Squadron located at NATO Joint Warfare Centre, Eikesetveien 29, 4032 Stavanger, Norway // When price quotations are submitted please note that quotes should be exclusive of Value Added Tax. // All contractor personnel and potential sub-contractors may be subject to a background investigation prior to gaining access to the 426th Air Base Squadron. //Offeror shall complete bid sheet at end of this solicitation. Offeror shall also submit resume or letter to demonstrate how they meet PWS qualifications in paragraph 1.1. //Offeror must be registered in System for Award Management (SAM) to be eligible for award. Go to www.sam.gov to register and for details on how to register. Please provide your DUNS and Cage code on the bid sheet. //Invoicing will be through Wide Area Work Flow (WAWF). Once SAM registration is complete, please go to https://wawf.eb.mil/ to register for WAWF. Successful offeror will only be paid once a valid invoice has been submitted through the WAWF system. The Government may require continued performance of any services within the limits and at the rates specified in the contract. OMBUDSMAN (AUG 2005) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution.
