Description
NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The Request for Quotation (RFQ) is VA786-13-Q-0112. No other solicitation will be issued in response to this announcement. This announcement will serve as both the synopsis and the solicitation. Quotes in response to this combined synopsis/solicitation are due February 13, 2013 by 2:00PM MT and shall be emailed to Derek.Reed@va.gov. Purchase order award will be made to the quoter/offeror whose quote is responsive, and offers the best overall value to the Government. Best Value will be determined based on past performance and program management. As a minimum quoters must provide the following information: 1) Past Performance, include references for at least three (3) clients similar in size, scope and complexity as the this requirement and include information as to the type of customer (federal, state, local, commercial), contract number, name of contracting officer or contracting officer representative, dollar value of contract, term of contract, active or closed, and telephone and fax numbers for points of contact for reference information. 2) Program Management: offerors/quoters shall explain how their workforce will be managed and provide information as to how they will respond to performance and/or quality issues. This RFQ document incorporates provisions and clauses which are in effect through Federal Acquisition Circular FAC 2005-61. The type of purchase order being contemplated is firm fixed. The associated North American Industrial Classification System (NAICS) code for this procurement is 561730 and the small business size standard is $7.0M. This requirement is set-aside 100% for Service Disabled Veteran Owned (SDVOSB) Small Business Concerns; and only qualified and eligible SDVOSB concerns may be considered for possible purchase order award. The period of performance is 365 calendar days from receipt of a purchase order award. The place of performance is Fort Bliss National Cemetery located at 5200 Fred Wilson Avenue, El Paso, TX 79906. Information regarding the date and time of the organized site visit is located within the specifications. The contractor shall provide both a unit price and total amount for each of the Contract Line Item Numbers (CLINS) attached, and all pricing shall be based on requirements of the statement of work. The deadline for receipt of questions is February 5, 2013, 2:00PM MT. Questions can be emailed to Mr. Reed at Derek.Reed@va.gov. Offerors/quoters must be registered in the System for Award Management (SAM) at https://www.sam.gov to be eligible to receive purchase order award. In addition, SDVOSB concerns must be registered and have an active account in www.vetbizregitery.gov to be eligible to receive an award. The selected Offeror must comply with the following commercial services terms and conditions, which incorporates by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. FAR 52.212-1, has been addendum as follows:1)Past performance will be assessed and rated based on an assessment by Government personnel as to the offeror/quoter's ability to successfully perform requirement based on past performance. Each offeror/quoter will be assigned a ratings of: High Confidence, meaning based past performance, essentially no doubt exists the contractor will successfully perform the required effort; Significant Confidence, meaning based on past performance, little doubt exists the contractor will successfully perform the required effort; Satisfactory Confidence, meaning based on past performance, some doubt exists the contractor will successfully perform the required effort; Little Confidence, meaning based on past performance record, substantial doubt exists the contractor will successfully perform the required effort; No Confidence, meaning based on the offeror's past performance record, extreme doubt exists the contractor will successfully perform the required effort; and Neutral/Unknown Confidence, meaning no relevant performance record is identifiable or relevant past performance was provided and an overall assessment rating of "Neutral/Unknown Confidence" which is not considered favorable or unfavorable will be assigned. 2)Program Management Approach - This factor will be rated Satisfactory or Un-Satisfactory, given how the contractor intends to satisfy requirements of the purchase order. Factors 1 and 2 are both considered equal in importance, and when combined, these factors are both considered significantly more important than cost or price. As such, the Government reserves the right to make purchase order award to other than the lowest priced offeror/quoter. FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5- Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items. The following clauses are included: 52.222-50, 52.219-27, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.232-30, 52.232-34, 52.222-41, and 52.222-26.
