Description

NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The Request for Quotation (RFQ) is VA786-13-Q-0109. No other solicitation will be issued in response to this announcement. This announcement will serve as both the synopsis and solicitation. Quotes in response to this combined synopsis/solicitation are due January 31, 2013 by 2:00PM MT and shall be emailed to Geraldine.Herrera@va.gov. Purchase order award will be made to the quoter/offeror whose quote is responsive, and offers the lowest aggregate price to perform the minimum quantities as specified in this announcement. This RFQ document incorporates provisions and clauses which are in effect through Federal Acquisition Circular FAC 2005-61. The type of purchase order being contemplated is indefinite quantity type, and the actual number of trees will be determined based upon funding available at time of award. The associated North American Industrial Classification System (NAICS) code for this procurement is 561730 with a small business size standard of $7.0M. This requirement is set-aside 100% for Service Disabled Veteran Owned (SDVOSB) Small Business Concerns; and only qualified and eligible SDVOSB concerns may be considered for possible purchase order award. The period of performance is seven (7) calendar days from receipt of a purchase order award. The place of performance where all work will be accomplished is the Black Hills National Cemetery located at 20901, Pleasant Valley Drive, Sturgis, SD 57785. Interested parties may contact the Cemetery point-of-contact, Terry Corkins at (605) 347-3830 to schedule a site visit. The contractor shall provide both a unit price and total amount for each of the following Contract Line Item Numbers (CLINS), and all pricing shall be based on the attached statement of work. The contractor shall provide all plant, labor, tools, parts, equipment, transportation and incidentals needed to perform the following: CLIN 001 - Prune, Trim, Cleanup and Dispose of Waste Debris for an estimated five (5) large trees over 40 feet in height. CLIN 002 - Prune, Trim, Cleanup and Dispose of Waste Debris for an estimated thirty-three (33) Medium trees 40 feet and under in height. CLIN 003- Stump Grinding/Removing Only. Grind Stump, Clean-up and Disposal of Waste Debris and remove from site for an estimated quantity of five (5) stumps. All quantities are estimated with the minimum quantity of work guaranteed to remove and/or trim being thirty (30), and the maximum quantity of trees to remove and/or trim and/or prune being sixty (60). Offerors/quoters must be registered in the System for Award Management (SAM) at https://www.sam.gov to be eligible to receive purchase order award. In addition, SDVOSB concerns must be registered and have an active account in www.vetbizregitery.gov to be eligible to receive an award. The selected Offeror must comply with the following commercial services terms and conditions, which incorporates by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5- Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items. The following clauses are included: 52.222-50, 52.219-27, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.232-30, 52.232-34, 52.222-41, and 52.222-26.
