- Description
- It is the intent of the Phoenix Area Indian Health Service to acquire quotations for a Firm-Fixed Unit Priced Non-Personal Service contract for on-site neurology physician services to be performed at the Hopi Health Care Center (HHCC) located in Polacca, AZ. In the performance of services, neurologist shall to provide one scheduled six-hour clinic at the HHCC on a quarterly basis (4 clinics per year). The anticipated period of performance is for a base year of twelve (12) months, beginning March 1, 2013 through February 28, 2014, with option to extend services for four (4) additional one-year option periods. The Contractor shall meet all criteria contained in the RFQ. The unit price shall be an all-inclusive cost to include travel, lodging, per diem, fringe benefits, malpractice insurance, federal, state and local taxes, including all other costs pertinent to the performance of the contract. Requests for copies of the quotation shall be made in writing via email address: sharon.cavanaugh@ihs.gov; provide your name, email address or mailing address and indicate how you prefer to receive a copy of this RFQ. The RFQ will be available within five (5) calendar days after the date of this publication. The closing date of the RFQ is February 12, 2013.
