- STATEMENT OF WORK FOR ASBESTOS AND OTHER HAZARDOUS MATERIALS ABATEMENT, AIR MONITORING AND INSULATION 1. SYPNOSIS: This statement of work is to provide the contractual services for Asbestos and other Hazardous Materials abatement, air monitoring and insulation for the Corpus Christi Army Depot, on as on call basis. The Contracting Officer's Representative (COR) shall identify the work and the contractor shall provide a proposal for the work. Based on the mutually agreed upon proposal, the contractor shall provide a detailed cost proposal (material and labor hours) which shall be reviewed and negotiated prior to the government approval to perform the work. The contractor shall then perform the work in accordance with that proposal. 2. SERVICES: All Contractor abatement work shall comply with the Occupational Safety and Health Administration (OSHA), Environmental Protection Agency (EPA) and National Emission Standards (NESHAP) requirements. The Contractor shall also comply with the requirements set forth in the CCAD Asbestos Project Requirements provided as Attachment 1. The Contractor will be trained and licensed under the requirements set by the Texas Department of State Health Services prior to performing services at the Naval Air Station (installation). The Contractor shall provide all labor, supervision, administration, and equipment (except as specified as Government furnished) to provide hazardous material abatement; air monitoring and insulation services to Corpus Christi Army Depot. Services shall be provided in accordance with all terms, exhibits contained herein or incorporated by reference. Incorporated by reference shall include any and all mandatory provisions of the Federal Acquisition Regulation (FAR), Defense FAR Supplement (DFARS), and Army FAR Supplement (DFARS). Upon contract award, the Contractor shall submit three (3) copies of their company safety and health plan. The Contractor shall provide the government with an abatement plan at least three (3) business days prior the commencement of onsite work. Also, the government must be supplied with the MSDS's for any glues, solvents, or any other chemicals to be brought on the installation prior to the commencement of any site work. All Contractor abatement work is considered complete and ready for payment upon receipt and approval of final close-out report. The final close-out report must be presented for approval by CCAD's Facilities Engineering Management Division and Safety and Environmental Division. At a minimum it must contain the following: 1.A narrative on the scope of work including specific location of the project, materials and quantities removed, project duration, dates of removal, dates of visual clearance/air clearance, and approved changes to the scope of work. 2.Competent person daily log reports, visitor, containment, and daily sign-in sheets, record of safety meetings, state notification(s), worker certification/licenses/medical surveillance info, air monitoring logs, laboratory sheets, and lab certification, if applicable. 3.Waste Manifests and MSDS for replacement parts and/or materials. 3. TURN AROUND TIME: a.The Contractor shall respond to a call or an abatement project within one business day of a call during normal hours Monday through Friday, government holidays exempted. b.The Contractor will make a best effort to respond to emergencies within 2 hours after notification. c.The Contractor shall perform the abatement within three days of receiving the appropriate clearances. 4. SITE PREPARATION: It shall be the Contractor's responsibility to provide certified design containment and put it in place for each job. 5. CLEAN UP: It shall be the Contractor's responsibility to clean up and police the work site. At the end of each working day, the Contractor's work site shall be left clean, orderly, and in hazard free condition. When the work is completed, the work site shall be policed to remove any pieces of wire, boxes, trash, debris, etc. 6. DEMOLITION: The Contractor, after performing the demolition shall be responsible for the complete removal and disposal of all demolition products from the Naval Air Station. All Federal, State, and Local rules and regulations applicable to the disposal of these demolished products shall apply. NOTE: The only exception to the above is if the demolition products will fit into a single 55 gal drum, then the Army will dispose of it through its own Hazardous Materials Systems. 7. SAFETY: The Contractor shall utilize good safety practices at all times to reduce the risk of fire, damage, contamination and personal injury. All work shall be performed utilizing all OSHA safety standards. The contractor shall be responsible for providing and placing barricades, air monitoring, warning signs, and any/all safety devices during work performance. In addition, lock out/tag out procedures will be strictly followed and enforced. All Contractor employees will wear protective equipment required in the area of work at all times. All work areas shall be roped/barricade to prevent any unauthorized entrance. 8. ACCESS TO NAS CORPUS CHRISTI: The Contractor's vehicle shall be identified as a contractor vehicle with the company logo plainly marked on it. NOTE: Company vehicles are permitted to enter the depot without government decals. 9. PARKING ON CCAD PROPERTY: The Contractor may park its vehicles anywhere within the normal CCAD employee parking. No parking is allowed within fire lanes or on handicapped spaces. 10. COOPERATION: The Contractor shall cooperate fully with the COR to accomplish work in support of the Depot and its assigned mission. In order to fulfill the intent of the contract, the Contractor must be flexible in meeting changing requirements and priorities. The Government reserves the right to re-prioritize the work to be done under the contract in order to meet changing needs and requirements., so long as such changes are within the services to be provided by the Contractor and do not require additional cost for the services rendered. 11. GOVERNMENT FURNISHED UTILITIES: Electrical power will be furnished by the government at existing outlets for the Contractor's use at no cost to the Contractor for accomplishing the work described in the Statement of Work. Electric power available is 110 volt, single phase, 60 cycle. The maximum load applied to a circuit shall not exceed 15 amperes. Failures of the government to furnish utilities at any time during the performance of the contract, due to outages or any interruptions in service shall not be used by the Contractor as a basis of claim against the government. Water will also be available for the contractor to use at no cost to the contractor although the government can not guarantee the proximity of the water access to the work site. 12. SECURITY: The Contractor shall insure all employees have a current and valid base pass when performing work on the installation. Contractor is responsible for the safe keeping and/or loss of repair part and any/all Contractor- owned equipment at the job site and /or during repairs. Contractor is responsible to have any/all insurance that is required. Please note that per Army regulations, all Contractor personnel must be U.S. Citizens. 13. FIRE PROTECTION: All welding and burning operations shall be accomplished in strict compliance with the National Fire Code and local safety regulations. Prior to welding, burning, and/or cutting operations, the Contractor shall obtain a burning permit from the Navy Fire Department on a daily basis. These permits are the only acceptable authorization for welding, cutting, heating, or burning. 14. EMERGENCY MEDICAL SERVICE: Medical services for the Contractor's personnel shall be the responsibility of the Contractor. However, the Government shall provide, on an emergency basis, medical services for on-the-job injuries/illnesses while an employee is performing under the contract. Emergency medical service will be of a first aid nature in order to allow the Contractor to contact his physician and transport the patient to doctor's office or hospital. Contractor shall reimburse the Government for emergency medical services provided on station upon receipt of invoice from the medical facility. 15. SUPPLIES: For all levels of work, the Contractor shall be responsible for buying, stocking, and distributing all material except as defined herein as government supplied.
