W-9 VENDOR ACKNOWLEDGEMENT FAR CLAUSES TERMS AND CONDITIONS NON PERISHABLE FOOD SERVICE BID Request for proposal Food Service Supplies The Iroquois Job Corps Center (IJCC) and Education & Training Resources (ETR); located at 11780 Tibbett's Rd., Medina, NY. 14103, are desirous of a fully insured supplier to submit a bid sheet on the federally funded project mentioned herein; and if successfully awarded the subject contract, to satisfactorily perform the following work project according to the established criteria contained within same. Bidders must have a Dun & Bradstreet (DUNS) number to be awarded a federal contract and provide documentation of such. Bidders must be registered in Central Contractor Registration (CCR) before being awarded a federal contract and provide documentation of such. Final 3 month perishable bids and non-perishable bids are due to this center, not later than 25 January 2013 at 4:00 PM. Perishable food bid list is due from participating vendors every week on Thursdays there after. Point of contact for this project is the IJCC Purchasing Department. Vendors are needed to provide and deliver food service items for the 250 +/- students of the Iroquois Job Corps Center. 2. All vendors must completely fill out, sign and return the "Food Service Supplies Bid Pricing Information Sheet" found on page two of this scope of work with their proposal. 3. All vendors must use the IJCC "Food Service Supplies Bid Sheet 01 FEB 2013 - 30 April 2013" (Bid Sheet "NON_PERISABLE BID SHEET -1" AND PERISHABLE BID SHEET -2) for individual pricing of Food service supplies, and submit it electronically to tina.burrage@etrky.com and sonya.rodriguez@etrky.com. All other required items can be submitted by at same time by fax, mail or sent by electronic email. Only Fill in Bid Price amounts and comment section. Do Not Change any other part of the bid excel format. 4. All items are to be bid as written. When a specific brand name and model/style is listed please advise in comment section of bid sheet the substitute of another product. Substituted items will be considered if it is a like quality and a clearly defined product item. Any product questions should be communicated by email to be directed to tina.burrage@etrky.com and sonya.rodriguez@etrky.com All materials are to be delivered to our location at 11780 Tibbet's Rd Medina, NY. 14103, by the successful bidder (s). 6. All deposits for any item must be included in the unit price. A separate charge for deposits will not be honored. 7. Specific delivery date and time will only be accepted on Wednesdays between 7:30 and 9:30 AM. Any Fuel/Delivery/Freight charges must be clearly written and within the bid. 8. Selection of the vendor (s) will be based on the lowest Fee-For-goods pricing for each item, from vendors that comply with this scope of work. This is NOT single source procurement. 9. If there is a tie on a line item price, the item will be awarded to the vendor that submitted the most complete line item bid. In compliance with the Bid Provision, Scope of Work, Terms & Conditions, and other specifications contained in this RFP, the undersigned hereby proposes to provide the materials listed for the center. Bid Price for period 1 Feb 2013 - 30 April 2013: The Rates herein are no higher than would be for any other customer, either commercial and/or agencies of the United States Government, for similar services. *Please use the IJCC "Vendor NON-PERISHABLE Food Bid Sheet 1 and PERISHABLE BID Sheet 2 01Feb 2013 - 30 April 2013" for pricing food items. Above rates include necessary labor, transportation, and supervision required to accomplish Scope of Work.
