- Base Notice: Notice of Intent to Award a Sole Source Contract - FDA1113881 Notice Information Solicitation: FDA 1113881 Agency/Office: Food and Drug Administration Location: Office of Acquisitions and Grants Services Title: Notification of Intent to Award a Sole Source Purchase Description(s): Notification of Intent to Award a Sole Source Purchase Order The Food and Drug Administration, intends to make a sole source award for a (1) year service agreement for the One (1) year service agreement for the Smart Cycler Real-Time PCR System, [Serial No.'s 401272, 200496, 403221, 900869, 403231 & 403414], to Cepheid of Sunnyvale, California in accordance with FAR Part 12 and the Simplified Acquisition procedures set forth in FAR Part 13.106. The resultant purchase order will include all applicable provisions and clauses in effect thought the Federal Acquisition Circular 05-50, NAICS Code 811219. The basis for this award is "only one responsible source". Other interested parties, who possess the required capabilities to provide the services outlined under the specifications, may also present information for consideration. This is for a servicing agreement that includes: Service and maintenance for the Smart Cycler Real-Time PCR System to be performed for one (1) Base Year to include four (4) option years of servicing for the Food & Drug Administration's, Office of Regulatory Affairs (ORA). During the term of the Service Agreement, the vendor will provide all parts (including I- CORE® modules to replace failed modules), labor and return shipping costs associated with the repair and/or calibration of the Instrument incurred during the contract period. I-CORE modules ordered as spares by the customer are not covered by the Service Contract. All repair work completed by the vendor's Service Representatives is warranted to have been performed in accordance with industry standards for the lessor of 90 days or for the duration of the existing Service Agreement. All replacement parts are warranted to be free from defects in workmanship. The projected award date is February 6, 2013. Period of performance: Base Year: February 18, 2013 thru February 17, 2014; Option Year One: February 18, 2014 thru February 17, 2015, if exercised; Option Year Two: February 18, 2015 thru February 17, 2016, if exercised; Option Year Three: February 18, 2016 thru February 17, 2017, if exercised; Option Year Four: February 18, 2017 thru February 18, 2018, if exercised; Primary Point of Contact.: Ricardo H.
