Page 1 of 3 JANUARY 16, 2013 SCOPE OFWORK STORM WATER MANAGEMENT POND IMPROVEMENTS 244 NEEDY ROAD MARTINSBURG,WV 25405 I. SCOPE OF WORK CONTRACTOR WILL PROVIDE THE NECESSARY SUPERVISION, LABOR, EQUIPMENT, MATERIALS, AND PERMITS FOR THE STORM WATER MANAGEMENT POND IMPROVEMENTS. THE POND IMPROVEMENTS PROJECT IS LOCATED AT THE ATF FACILITY LOCATED AT 244 NEEDY ROAD, MARTINSBURG, WV 25405. A. PROVIDE ALL NECESSARY PERMITS PRIOR TO MOBILIZING TO THE JOBSITE. B. PROVIDE A PROJECT CPM SCHEDULE PRIOR TO MOBILIZATION. C. REMOVE FENCE AND INSTALL TEMPORARY GATE. D. CLEAN EXISTING 15" CMP UNDER ROAD, APPX. 30 LNFT. E. INSTALL RIP RAP AT EXISTING 15" CMP - 2 TONS. F. EXCAVATE THE POND FOR PLUMBING. G. INSTALL DIRT AND SEDIMENT BAGS TO PUMP THROUGH TO COLLECT THE SEDIMENT. H. CLEAR AND GRUB POND'S PERIMENTER. I. FURNISH AND INSTALL APPROPRIATE STORM WATER MANAGEMENT POND STRUCTURE WITH DEBRIS RACK. J. REGRADE SLOPES FOR BERMS AND DAMS. K. INSTALL RIP RAP AT THE 3 EXISTING PARKING LOT DRAINS, APPX. 300 LNFT. L. SEED AND MULCH TO STABILIZE ENTIRE SITE. M. REINSTALLATION OF THE SECURITY FENCE. N. THE SERVICEWILL CONSIST OF PUMPING, DEWATERING, REMOVAL AND DISPOSAL OF SLUDGE, TREE BRANCHES, LEAVES, SHRUBS AND ALL OTHER DEBRIS ASSOCIATED WITH THIS OPERATION. O. THE CONTRACTOR WILL PROVIDE POLYMER, CONVEYERS, TRACTORS, TRAILERS, BOXES, AND ANY OTHER EQUIPMENT NECESSARY FOR THE OPERATION AND DISPOSAL. P. THE ESTIMATED DURATION FOR THIS SERVICE IS 45 DAYS; HOWEVER IT MAY GO LONGER DUE TO UNFORESEEN CONDITIONS OR INCLAMENT WEATHER. Page 2 of 3 JANUARY 16, 2013 Q. THIS OPERATION WILL BE DONE DURING REGULAR HOURS; UNLESS OTHERWISE COORDINATED WITH THE GOVERNMENT. P. THE SUCCESSFUL BIDDER MUST DISPOSE OF ALL SLUDGE, TREE BRANCHES, LEAVES, SHRUBS, AND ALL OTHER DEBRIS ASSOCIATED WITH THIS OPERATION. Q. TRANSPORTATION AND COSTS TO BE BORN BY THE CONTRACTOR. R. LANDFILL TIPPING FEES TO BE BORN BY THE CONTRACTOR. II. EACH BIDDER SHALL PROVIDE: A. ITEMIZED COST BREAKDOWN. 1. PERMITTING 2. MOBILIZATION / DEMOBILIZATION 3. REMOVAL FENCE / TEMPORARY GATE 4. DEWATER POND 5. CLEAR & GRUB POND PERIMETER 6. DREDGE POND & REMOVE EXCESS 7. INSTALL SWM POND STRUCTUREWITH DEBRIS RACK 8. REGRADE SLOPES, BERMS, AND DAMS 9. RIP-RAP DITCH 10. SEED. MULCH, AND STABILIZE SITE III. GENERAL: A. EACH BIDDER IS ALLOWED TO INSPECT THE JOB SITE PRIOR TO BIDDING. BIDDER SHOULD VERIFY ABILITY TO LOCATE EQUIPMENT. B. THE BIDDER SHALL SUBMIT REFERENCE DATA ON THE PROPOSAL FORM OF THREE PROJECTS MINIMUM, OF SIMILAR COMPLEXITY, TYPE, AND COMPARABLE VALUE. C. ALL ONSITE LABOR AND SUPERVISION SHALL BE OF U.S. CITIZENSHIP. D. FAILURE TO PROVIDE ALL REQUIRED INFORMATION OR LACK OF A FAVORABLE REFERENCE MAY BE GROUNDS FOR REJECTION OF BID. IV. SAFETY: A. OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION (OSHA) STANDARDS SAFETY PRACTICES, RULES AND INDUSTRIAL GUIDELINES SHALL BE CONSIDERED PART OF THESE SPECIFICATIONS, AND SHALL BE ADHERED TO BY ALL. THE WORK MAY INVOLVE EXPOSURE TO POTENTIALLY BIOLOGICALLY HAZARDOUS SUBSTANCES AND APPROPRIATE PROTECTIVE CLOTHING AND GLOVES SHOULD BEWORN. B. THE CONTRACTOR SHALL TAKE STEPS NECESSARY TO FULLY COMPLY WITH ALL APPLICABLE LOCAL, STATE AND FEDERAL REGULATIONS, INCLUDINGWVDEP PERMITTING REQUIREMENTS. Page 3 of 3 JANUARY 16, 2013 IV.
