- Quotations will be made via email to debra.a.smith@uscg.mil. No other method of submission will be accepted. The following FAR Clauses/Provisions apply to this solicitation. Offerors may obtain full text version of these clauses electronically at www.arnet.gov/far. FAR Clause 52.204-6, 52.204-7, 52.212-1, 52.219-1, 52.219-6, 52.222-3, 52.222-19, 52.222-16, 52.222-26, 52.232-33, 52.222-41, 52.222-44, 52.223-6, 52.232-33. The following Homeland Security Acquistion (HSAR) are incorporated as an addenda to this solicitation. HSAR305.209-70, 3052.237-72. Copies of the HSAR may be obtained electrically. Vendor's quotes must contain the following, otherwise you quote will not be comsidered: 1) Solicitation number 2) A complete description of supplies or services that are being requested by the USCG 3) Vendor name, address, city/state, phone number, email address 4) A complete copy of the representations and certificates FAR 52.219-1 5) Vendors tax ID along with their DYNS number 6) Vendors providing an offer MUST be registered in the SAM system (System for Award Management) prior to the closing date on this solicitation. Vendors must insure that the above NAICS codes are listed on their SAM registration to perform this type service Vendor quotes should include the following (if there is anything missin or need Statement of Work: Prepare and Deliver All-Hazards Incident Leadership Course (L-381) for MSU Savannah's Type III PQS requirements 1. Scope of Work. 1.1. The contractor will provide training in Incident Leadership, consisting of one session, five days in duration, for up to 18 participants. Course content will be tailored to the needs and specifications of the target audience, MSU Savannah and Sector Charleston personnel to complete a required Type III PQS requirement. 1.2. Services/Items to be included are as follows: 1.2.1. Pre-work necessary to customize the course to Coast Guard-specific missions and contingencies. 1.2.2. Distribution of a pre-training work assignment that will be sent individually to all course participants 1.2.3. Three course trainers for the duration of the entire course, each possessing specific experience in human factors of psychology, emergency response operations, and military leadership/doctrine 1.2.4. Instructor travel, lodging and subsistence to, from and at the training site 1.2.5. Teaching aids and presentation equipment as needed 1.2.6. One complete set of instructional materials for each person enrolled in the course 1.2.7. Transportation of all course materials and instructional equipment to and from the training site 1.2.8. Post work including provision of graduation certificates to each participant and evaluation of course critiques 1.3. Services/Items that are NOT included are as follows: 1.3.1. Salary and other related costs associated with MSU Savannah & D7 logistics support 1.3.2. Facility costs 1.3.3. Participant travel, lodging and subsistence to, from and at the training site 1.3.4. Field equipment that the participants may require 1.3.5. Contractor travel expenses for client-requested meetings regarding course customization, if the meeting(s) occur outside the Denver, CO area. 1.3.6. Travel, lodging and subsistence for other than contractor employees 1.3.7. Compensation for course participants in any after-hours training 1.4. The contracting agency will provide the following: 1.4.1. A suitable facility at a site to be determined, in the eastern U.S., by MSU Savannah and D7. 1.4.2. A person to serve as logistics coordinator, who will be responsible for the following: 1.4.2.1. Arranging for the training facility and support items 1.4.2.2. Making hotel room block arrangements as needed 1.4.2.3. Finding and securing areas for training as needed 1.4.2.4. Arranging transportation for participants to the training 1.4.2.5. Purchasing and shipping classroom supplies 1.4.2.6. At the training site, assisting the contractor with classroom preparation; participant check-in, attendance and graduation; 1.4.3. A person to serve as technical expert, providing substantive information and technical guidance as needed for the contractor to customize the course 2. Period of Performance. 2.1. Contract and service delivery will commence 15 April 2013, Savannah, GA area. 2.2. Contract and service deliver will conclude 19 April 2013, Savannah, GA area. 3. Deliverables 3.1. Deliverable 1 - Final Lesson Plan 3.1.1. Minimum Requirements 3.1.1.1. Outline of course content, specific to Coast Guard missions. 3.1.1.2. Complete package of all teaching/visual aids that will be presented in the course. Can be printed or in digital form. If in digital format, must be compatible with MS PowerPoint software. 3.1.1.3. List of presentation equipment the contractor will use. 3.1.1.4. List of resources MSU Savannah & D7 will need to provide. 3.1.1.5. List of logistics support MSU Savannah & D7 will need to provide. 3.2. Deliverable 2 - Participant Instructional Materials 3.2.1. Minimum Requirements 3.2.1.1. One complete set of instructional materials that will be provided to all course participants at the site of instruction. Must be in printed form. 3.3. Deliverable 3 - Presentation of Course 3.3.1. Minimum Requirements 3.3.1.1. Five consecutive days of instruction for 18 people. 3.3.1.2. Instruction time not less than 40 hours. 3.3.1.3. Instruction real time and in person by three or more qualified instructors. 3.3.1.4. Teaching methods involve appropriate use of teaching/visual aids. 3.3.1.5. Delivery and use of one complete set of instructional materials to each participant in the course. 3.4. Deliverable 4 - Final Invoice for Payment 3.4.1. Minimum Requirements 3.4.1.1. Submit invoice for payment to Contracting Activity. 4. Milestones 4.1. Milestone 0 - No later than 25 February 2013, D7 will provide the contractor with a notice to proceed under an approved contract. 4.2. Milestone 1 - No later than 15 March 2012, the contractor will teleconference with the D7 technical expert for the purpose of identifying modifications that are needed to customize the course to meet the specific needs of the MSU Savannah and Sector Charleston personnel to complete a required Type III PQS requirement. 4.3. Milestone 2 - No later than 15 January 2013, MSU Savannah & D7 will select a location/city for delivery of the course of instruction. 4.4. Milestone 3 - No later than 15 January 2013, the contractor will provide MSU Savannah & D7 with a written proposal for specific course content. 4.5. Milestone 4 - No later than 01 February 2013, MSU Savannah & D7 will select and secure a specific facility where the instruction will be delivered. 4.6. Milestone 5 - No later than 30 March 2013, the contractor will provide a draft Participant Instruction Material package for MSU Savannah & D7 review. 5. Cost and Payment 5.1. This is a fixed-price contract with a value of $24,900. If paid prior to 06 June 2013 the fixed-price total will be $24,000. 5.2.
