Description
ACC-APG Huachuca Division at Fort Huachuca, AZ Notice of Intent to Award on a Lowest Price Technically Acceptable Basis. This is a Notice of Intent to award on a Lowest Price Technically Acceptable (LPTA) Basis for Grease Trap Services as described in accordance with the attached Performance Work Statement. THIS IS NOT A REQUEST FOR PROPOSAL OR QUOTE. A FORMAL SOLICITATION WILL BE POSTED AT A FUTURE DATE. NO REIMBURSEMENT WILL BE MADE FOR ANY COSTS ASSOSCIATED WITH PROVIDING INFORMATION IN RESPONSE TO THIS SYNOPSIS OR ANY FOLLOW-UP INFORMATION REQUESTS. 1) The United States Army Contracting Command, Aberdeen Proving Ground, (ACC-APG), Fort Huachuca Division at Fort Huachuca, Arizona has a requirement for pumping and cleaning of grease trap systems as listed in the attached Performance Work Statement. Grease Trap Cleaning Services are described as removing all grease, dirt, or any other foreign material from grease traps. 2) The Solicitation will be competed as a Total Small Business Set-Aside in the near future. The Directorate of Public Works (DPW) provides support for the operation and maintenance of post facilities and requires systems to be serviced and maintained in a ready state for operation. 3) A site visit will be offered approximately 1 week after the solicitation is made available to bidders. Notification of the Site Visit date and time will be provided in the Solicitation document. 4) Any contract with the government requires registration in the System for Award Management (SAM) website url: https://www.sam.gov/portal/public/SAM/ in accordance with FAR Clause 52.212-3 -- Offeror Representations and Certifications; Offeror Representations and Certifications -- Commercial Items (Feb 2009) 5) Tax ID is required. 6) The period of Performance will be for One Base Year and Four Option Years Subject to Availability of funds under the authority of FAR 52.232-18. The vendor selected must be able to perform the services at Ft. Huachuca within 30 days of contract award for one year as specified in the Performance Work Statement. 7) See the attached Performance Work Statement for details of the Grease Trap Cleaning Services requirement. PERFORMANCE WORK STATEMENT (Grease Trap Services) 26 September 2012 A. PURPOSE A.1.The purpose of this contract is to remove all grease, dirt, or any other foreign material from Grease Traps. The term of the contract is for one year, with the option for renewal for each of the following four years. B. THE CONTRACTOR SHALL PROVIDE TRAINED EMPLOYEE(S) TO: B. 1. Provide a competent person to oversee all aspects of the work being preformed, safety, quality control, disposal of waste materials, and clean-up. B.1.2 The manager, competent person and alternate must be able to read, write, speak, and understand the English language. C. GENERAL. C.1. SCOPE OF WORK The contractor shall furnish all labor, materials, equipment, personnel, supervision and transportation to perform all operations in connection equipment to provide cleaning service for all grease traps listed at Section C.5.1. in accordance with paragraph C.5 with these specifications during scheduled hours. All work areas shall be left clean after completion of work, and all debris shall be removed. C.1.1. Replace Items. All defective components, parts, and materials shall be reported in writing to the Contracting Officer's Representative (COR). Any items damaged by the Contractor shall be replaced by the Contractor at no cost to the Government. C.1.2. Location of work is at Fort Huachuca, Arizona C.2. DEFINITIONS. C.2.1. Normal Work Hours. All work shall be conducted during the hours of 7:00 a.m. to 3:30 p.m., Monday through Friday, unless the Contracting Officer approves other hours. C.2.2. Quality Assurance. These actions taken by the Government to assure it is receiving the services for which it has contracted. The Contracting Officer's Representative (COR) or designated representative shall evaluate these services acting as Quality Assurance Evaluator (QAE). C.2.3. Quality Control. Those actions taken by the contractor to assure the Government receives the services for which it contracts and those services meet the requirements of the contract. C.3. GOVERNMENT-FURNISHED ITEMS. C.3.1. The Government will furnish water and electricity as required for the work to be performed. C.4. CONTRACTOR-FURNISHED ITEMS. C.4..1. Contractor shall provide all equipment or material required to complete job as stated in paragraph C.5. C.4.2. Certification. Contractor must provide proof that the firm has been engaged in inspection and maintenance services for the aforementioned items and has the qualifications both in terms of expertise and having proper and adequate equipment to accomplish the work as required. C.4.3. Material Safety Data Sheets (MSDS). Contractor shall provide MSDS to the COR or designated representative 10 days prior to start of work. Material Safety Data Sheets are required for all hazardous material. Contractor is responsible for complying with all federal and state environmental and safety requirements. A specific requirement is hazardous communication and MSDS in accordance with 20 Code of Federal Regulations Part 1910. Contractor is also responsible for proper management of hazardous materials and petroleum products under Arizona Revised Statutes, Title 49, and the Resource Conversation and Recovery Act, 49 Code of Federal Regulations Parts 260 through 279. C.5. SPECIFIC TASKS. C.5.1. The contractor shall at 60-day intervals perform bi-monthly maintenance on all grease traps located at buildings listed below and at the frequency indicated, this should be 6 times a year. All grease traps shall be cleaned bi-monthly by the third Friday of that month. Contractor shall contact the QAE two days prior to service being performed and, in addition, will contact the QAE when contractor physically arrives on Fort Huachuca to pump grease traps. The contractor will also be required to provide emergency on call services for all facilities listed. The contractor shall pump out all solid waste contents of the grease traps until empty. C.5.1.2. The contractor shall (where possible) scrape the walls of the tank to remove building-up of solids. C.5.1.3. The contractor shall secure all lids to the tank and make sure that they are fitted property before departing from the area. C.5.1.4. The contractor when finished shall clean the area of all trash and grease spills before departing from the work area C.5.2. Emergency Service. C.5.2.1. When it is determined by the COR there is a valid need for emergency cleaning of grease traps, the contractor shall respond within 2 hours of callout. C.5.3. SAFETY. C.5.3.1. Procedures. The work performance shall be in accordance with the following current publications: quote mark Corps of Engineers Manual, General Safety Requirements, EM 385-1-1; Occupational Safety and Health Act (OSHA) Public Law 91-596 and the standards as set by the National Board of Fire Underwriters (National Fire Codes) and NFPA National Fire Code, No. 91. Flammable solvents or other flammable cleaning aids shall not be used. During cleaning, proper ventilation shall be provided and safety precautions taken when cleaning is done inside Grease Trap tanks C.5.3.2. Contractor personnel shall use proper safety equipment at all times during the performance of this contract and shall perform all tasks in a safe manner. C.5.4. INSPECTION OF SERVICES. C.5.4.1. Inspection. The work shall be subject to inspection at any and all times. The presence or absence of the inspector shall not relieve the contractor from strict compliance with these specifications nor shall the presence of an inspector relieve the contractor from any deficiency that may be noted in the final inspection. Upon completion of work on each grease trap or other item required to be cleaned in each building, the contractor shall notify the COR or designated representative and a joint inspection shall be made with the contractor. C.5.5. Contractor Manpower Reporting. The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor shall report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor must create an account upon entering the site and is required to completely fill in the required information at the CMRA website: https://cmra.army.mil. Reporting period will be the period of performance not to exceed 12 months ending 30 September of each government fiscal year and must be reported by 31 October of each calendar year. C.5.6. Contract Payments. The contractor shall submit monthly invoices. Invoices shall be entered in Wide Area Workflow (WAWF) in order for payments to be processed. C.6.
