Evaluation Process. a. PERFORMANCE BASED WORK STATEMENT VETERINARIAN SERVICES 1. GENERAL. The contractor shall provide the services of a Veterinary Services for Department of Clinical Investigations (DCI) at Dwight D. Eisenhower Army Medical Center (DDEAMC). 1.1. DESCRIPTION OF WORK. 1.1.1. This is a non-personal services contract for Veterinarian Services. All Veterinary duties will be performed in accordance with the written AAALAC Program Description and Laboratory Animal Support Services (LASS) Standard Operating Procedures (SOPs). The contract veterinarian will provide veterinary support services in support of the DCI research, training and education missions. Specific services may include: Providing daily care and management of the laboratory animal program including, but not limited to: provision of routine and emergency veterinary medical and surgical care, animal protocol support, animal record maintenance, ensure compliance with animal welfare laws, regulations, and SOPs. Animal research protocol and amendment review; Consultation on protocol development and support; Assistance with animal research; regulatory questions/issues; Serve as alternate member for the Institutional Animal Care and Use Committee (IACUC); Available to Department of Clinical Investigation staff and IACUC by phone/e-mail. 1.1.2. Contractor service personnel shall read, understand, fluently speak and legibly write English. 1.2. DEFINITIONS Association for Assessment and Accreditation of Laboratory Animal (AAALAC) Laboratory Animal Support Services (LASS) Standard Operating Procedures (SOPs) Institutional Animal Care and Use Committee (IACUC) Department of Clinical Investigations (DCI) Dwight D. Eisenhower Army Medical Center (DDEAMC) 1.3. HOURS OF PERFORMANCE. 1.3.1. Contractor will perform up to an average of five (5) hours per week. Normal duty hours are Monday through Friday 0730-1600 (excluding holidays) at Department of Clinical Investigation Research Laboratory (Bldg. 38705). For any shift exceeding six hours, the contractor's employees shall be entitled to one half hour break. This break shall be non-chargeable to the government for compensation. Example: For a shift 0800 p.m. to 1630 p.m. compensation for pay purposes would be eight hours, with one half hour break. Contractor will respond to animal care related emergencies at the Department of Clinical Investigation Research Laboratory between the hours of 1600 and 0730 on Monday-Friday and any time on holidays and weekends as needed during the absence of the DDEAMC Attending Veterinarian. 1.3.2. Unscheduled absences. Unscheduled absences shall be called into the OIC or designated representative by contract personnel prior to or within one hour of the scheduled duty day that he/she is unable to report to work. 2. CONTRACTOR FURNISHED PROPERTY. The contractor shall furnish everything required to provide Veterinary services in accordance with this contract. 3. SPECIFIC TASKS. Veterinarian shall be responsible for: 3.1. Providing experienced laboratory animal veterinary coverage for an Association for Assessment and Accreditation of Laboratory Animal (AAALAC) accredited facility. 3.2. Reviewing new animal use protocols for suitability. 3.3. Monitoring existing animal use protocols for compliance. 3.4. Serving as the veterinary representative on the DDEAMC animal care and use committee as needed. 3.5. Monitoring animal welfare in the colony and recommending treatment as needed. 3.6. Providing direct oversight on animal surgeries and complex procedures. 3.7. Maintaining appropriate records of the above duties. 3.8. Maintaining professional competency involving clinical skills and knowledge of laboratory animal medicine. 3.9. Providing weekend and holiday animal care as needed. 4. PERFORMANCE MEASURES 4.1. Outcome. The contractor shall provide qualified personnel who will fulfill the requirement to provide veterinarian services at a standard enabling the MTF to provide such services at a high level of quality. 4.2. How Measured. The government will evaluate the contractor's performance under this contract using the method of surveillance specified in Technical Exhibit One. All surveillance observations will be recorded by the government. When an observation indicates defective performance, the COR will obtain the contractor's representative's initials on the record of the observation. 5. QUALIFICATIONS 5.1. Veterinarian performing on this contract must possess and maintain a current, active, valid, and unrestricted Veterinary license from the state of Georgia. 5.2. Minimum of two years full-time experience as a laboratory animal veterinarian or board certification in laboratory animal medicine. 5.3. Possess knowledge of laws and regulations pertaining to animal use and experience in providing veterinary care to multiple commonly used laboratory species. 6. Contractor Manpower Report. Contractor Manpower Report: The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address https://cmra.us.army.mil/ <https://cmra.us.army.mil/>. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer's Technical Representative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including sub-contractor); (6) Estimated direct labor dollars paid this reporting period (including sub-contractor); (7) Total payments (including sub-contractor); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); (9) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (10) Locations where contractor and sub-contractors perform the work (specified by zip code in the United States and nearest City, Country, when in an overseas location, using standardized nomenclature provided on website); (11) Presence of deployment or contingency contract language, and, (12) Number of contractor and sub-contractor personnel deployed in theater this reporting period (by country). (13) As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year. The contractor shall notify the Contracting Officer's Representative (COR) and the Contracting Officer by the 5th working day of November whether or not they have completed this report. TECHNICAL EXHIBIT PLAN 1 QUALITY ASSURANCE SURVEILLANCE PLAN PERFORMANCE OBJECTIVE PWS PARA PERFORMANCE MEASURE PERFORMANCE THRESHOLD 1. Education 1.1.2.
