Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. Simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of FAR Parts 12 and 13. This announcement constitutes the only solicitation issued. Quotations are being requested and a separate written solicitation will not be issued. Request For Quotation (RFQ) VA69D-13-Q-0561 entitled Rhinolaryngeal Stroboscope is issued as an unrestricted procurement. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-65. No telephone quotes will be accepted. NAICS code 339112 applies with the small business size standard of 500 employees. Offeror's quotes are hereby being requested and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of person authorized to submit quotes on behalf of the contractor. Contractor shall provide the following Kaypentax Brand Name Only items: Item 1: EPK-1000 Video Processor Kaypentax Model EPK-1000 1 each. Item 2: High Definition Camera, 3-CCD Kaypentax Model 9213HD 1 each. Item 3: Rhinolaryngeal Stroboscope Kaypentax Model 9400 1 each. Item 4: High Definition Digital Video Capture Module Kaypentax Model 9310HD 1 each. Item 5: Nasolaryngoscope 90K Series Kaypentex Model VNL-1190STK 1 each. Item 6: Rigid Endoscope Kaypentax Model 9106 1 each. Item 7: 22-37 mm Zoom Lens Coupler Kaypentax Model 9118B 1 each. Item 8: Printer, Color Kaypentax Model 9264C 1 each. Item 9: Endoscope Adapter Kaypentax Model 9122B 1 each. Item 10: Precision Cart (with mod kit) Kaypentax Model 80233-1 1 each. Item 11: Monitor Arm Kaypentax Model 80228 1 each. Item 12: Setup and Training Kaypentax Model 9198 1 each. Contractor shall provide two (2) operators manuals and one (1) service manual for each piece of equipment. Contractor shall provide a KayPentax Digital Video Stroboscopy System for the Audiology and Speech Pathology Department located at the Edward Hines VA Hospital, 5000 S 5th Avenue, Hines, IL 60141. The Contractor shall provide all components necessary for a complete operational Digital Video Stroboscopy System and will include, but not be limited to the following: Video Processor; HD Camera with HD Digital Capture Module; Rhinolaryngeal stroboscope; Nasolaryngoscope with color CCD chip tip; Rigid Endoscope; Lens coupler (22-37 mm zoom); Endoscope adapter; Precision cart for storage; 1920 x 1080 pixel resolution of recorded video; 1920 x 1200 pixel resolution HD video monitor (24'); Camera head dimensions: 32.6 mm x 38.6 mm x 41 mm; Video inputs: 1 component video input (HD), 2 S-Video inputs (Standard Definition); Compatible with rigid endoscope (HD) and distal-chip and fiber flexible endoscopes (Standard Definition). Options-DICOM compatible. Electrical-13A/125V. Features-Excellent image quality for stroboscopic and endoscopic exams, Instant retrieval and playback of recorded procedures, Ideal for laryngeal endoscopy, Report generator containing images and key exam findings, Standard video formats for portability of exam segments, Patient database (SQL) and exam management system, and Still image creation and MPEG DVD burning capabilities. Kaypentax Factory authorized setup and training on system to consist of assembling the entire system into a fully functionally operational system and a minimum of a full day of training for 9 clinicians on the system. Contractor shall provide FOB Destination Delivery within _________ days after receipt of order. Contractor shall be an Original Equipment Manufacturer (OEM) authorized dealer, authorized distributor or authorized reseller for the proposed equipment/system such that OEM warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions. Contractors shall submit the original documentation from the manufacturer on the manufacturer's official letter head stating that they are an authorized distributor for the specific items being procured. This procurement is for new equipment ONLY; no remanufactured or 'gray market' items. All Equipment must be covered by the manufacturer's warrant. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer is the most advantageous to the government, price and other evaluation factors considered. Offeror must register in SAM and provide DUNS number for verification.
