- IMPORTANT INFORMATION SOLICITATION - FA5587-13-T-0004-OPERATING ROOM NURSE It is important that your company reads, understands and submits a quote that is current, accurate and complete. In the past we have received incomplete/inadequate quotes and proposals from contractors. You will only have one opportunity to submit a complete quote, please ensure you fully address the requirements in the addendum 52.212-1 and 52.212-2. - This is a Firm Fixed Price (FFP) Contract and No Conditional Proposals will be accepted, ref FAR 16.202-1 and FAR 15.101-2, solicitation addendum 52.212-1 pages 14-17 and addendum 52.212-2 pages 17-19. - Ensure you submit FFP prices for the Basic Year and Four Option Years, solicitation pages 3-5. - Ensure you read and follow the guidance in the solicitation, specifically addendum 52.212-1 pages 14-17 and addendum 52.212-2 pages 17-19. - Ensure you submit all of the required information for all the factors and sub-factors, addendum 52.212-1 and addendum 52.212-2. - Ensure that all copies of degrees, licenses and certifications are clear, readable and current. Please do not send in photographs of framed degrees as these are extremely difficult to read. - Ensure you follow the instructions to submitting your proposal before the due date and time as identified in addendum 52.212-1, solicitation pages 14-17. *This is informational guidance to potential contractors. This does not replace the requirements of the solicitation.
