Description

Bidders are encouraged to check often for changes (AMENDMENTS) to the solicitation. This solicitation number W9127Q-13-R-0002 has been modified to reflect the current changes: No paper copies of the solicitation will be mailed and telephone requests for paper copies will not be honored. Prospective firms should provide the following information: (1) Identification of company size status, certifications, i.e., 8(a), HUBZone, SDVOSB, WOSB, EDWOSB (2) Provide Specifications and or complete salient description for both the 96k lb container handler and 35k lb forklift. (3) Responses shall be submitted electronically via e-mail addressed to angela.d.duren.mil@mail.mil or by fax 601-313-1656 in the form of the attached 1449 (W9127Q-13-R-0002 Modified Solicitation), February 28, 2013 4:00pm CST. Proposal shall include (broken down into each item) monthly rental, all freight, warranty, dealer set-up fees and installation at Camp Shelby, Mississippi. Sign the front page of the solicitation; Fill out block 17a. to include your company's name, address, telephone number, point of contact. Also, Fill out block 26 to reflect the total amount and sign block 30a, 30b and 30c. (4) Include the numbers of days required to make delivery after receipt of purchase order. (5) Include CAGE Code number, DUNS number and TAX ID. (6) Acknowledge receipt of attached amendment with submission by returning a signed copy of the amendment, SF30. Sign block 15a, 15b and 15c (W9127Q-13-R-0002-0001 Modification) (7) Required information listed above must be submitted in order for your proposal to be considered. Failure to provide this information will result in your proposal as being non-responsive. Proposals recieved after the deadline will not be considered. CLIN 0001: LEASE OF 96K CONTAINER HANDLER, 1EA. The Contractor shall provide equipment and maintenance services for the leased equipment. All work shall be accomplished in accordance with the best commercial practices for forklift services. Contractor will provide equipment that is fully functional with no deficiencies. Unit shall be delivered fully serviced and ready to be put into operation. Contractor will be required to provide operator orientation training on the unit. CLIN 0002: Preventative Maintenance 2 EA. Charge is per every 500 hours of usage. At a minimum, two scheduled visits for preventative maintenance are required during the lease of this vehicle. Base period will be from actual date of delivery through the following 12 months, with four option years. CLIN 0003: Option Year One Container handler Reach Truck (12 months). CLIN 0004: Option Year One Preventative Maintenance (12 months). CLIN 0005: Option Year Two Container handler Reach Truck (12 months). CLIN 0006: Option Year Two Preventative Maintenance (12 months). CLIN 0007: Option Year Three Container handler Reach Truck (12 months). CLIN 0008: Option Year Three Preventative Maintenance (12 months). CLIN 0009: Option Year Four Container handler Reach Truck (12 months). CLIN 0010: Option Year Four Preventative Maintenance (12 months). CLIN 0011: Return Charge- Container handler Reach Truck CLIN 0012: Lease of 35k Forklift, 1 EA. The Contractor shall provide equipment and maintenance services for the leased equipment. All work shall be accomplished in accordance with the best commercial practices for forklift services. Contractor will provide equipment that is fully functional with no deficiencies. Unit shall be delivered fully serviced and ready to be put into operation. Contractor will be required to provide operator orientation training on the unit. CLIN 0013: Preventative Maintenance 5 EA. The Maintenance includes greasing, oil, and filter changing. This line item is not definitized. Quantity and funding will be placed on this item as needed. Government will be charged to make repairs to the vehicle damaged while under the terms of the contract. Base period will be from actual date of delivery through the following 12 months, with Four option years. Equipment shall be delivered to Camp Shelby, Mississippi. CLIN 0019: Option Year Three Preventative Maintenance(12 months). CLIN 0020: Option Year Four 35k Forklift (12 months). CLIN 0021: Option Year Four Preventative Maintenance (12 months). CLIN 0022: Return Charge- 35k Forklift Proposals are due no later than 4:00 p.m., CST, 28 February 2013.
