- Attachment #1 Attachment #2 THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION AND A SEPARATE SOLICITATION WILL NOT BE ISSUED. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR 13.5. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-65 (29 Jan 2013). This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The solicitation number for this acquisition is HHSN002105 and is being issued as a Request for Quote (RFQ). The NAICS Code for this solicitation is 811219 and the Small Business Size Standard is $19.0 mil. This action is being solicited as a Total Small Business Set-aside. Introduction Background Currently within the Clinical Center Complex (defined as Warren Grant Magnuson Bldg (Bldg 10), Ambulatory Care Research Facility (ACRF) and Clinical Research Center (CRC), there are five (5) locations where sterilizer equipment is in use (See Attachment 1 for Location, Manufacturer, serial number and model number of the equipment). These sterilizers are critical for each of the locations as their operations according to design specifications protects the safety and health of patients, and ensures the accuracy and certification processing of laboratory data and equipment. Hence, it is critical that the vendor be fully capable of maintaining and repairing each of these types of sterilizers to meet or exceed manufacturer's specifications. The contractor must be headquartered or have service locations within the Washington Metropolitan area due to onsite emergency support service required. The contractor shall provide scheduled preventative maintenance and equipment repair or replacement, including documentation and reporting of all sterilizer equipment. Scheduled preventative maintenance and repairs must be completed according to each sterilizer's manufacturers' specifications. All work shall be performed in compliance with all Federal, state (Maryland) and local laws. In addition, work must meet equipment manufacturers' and generally accepted industry standards of good workmanship. The Clinical Center reserves the right to thoroughly inspect and investigate the establishment, facilities, equipment, business reputation, and other qualifications of any bidder, and to reject any and all offers irrespective of price, that shall be administratively determined by the Contracting Officer to be lacking in any essentials judged necessary to assure acceptable standards of performance on this contract. Provide the resumes of key personnel, especially the personnel conducting on-site preventive maintenance and repairs, demonstrating that qualified manufacturer trained personnel are available and will be assigned to this contract. Personnel proposed are to be vetted to ensure that staff can meet the requirements for NIH security clearance, including background check and fingerprinting. Period of Performance Base year: April 1, 2013 through March 31, 2014 Option Year 1: April 1, 2014 through March 31, 2015 Option Year 2: April 1, 2015 through March 31, 2016 Option Year 3: April 1, 2016 through March 31, 2017 Option Year 4: April 1, 2017 through March 31, 2018 Statement of Work (SOW) (Attachment 1 ) ***Statement of Work (SOW) Attached (see attachment for detailed) Equipment List (Attachment 2) ***Equipment list is Attached (see attachment for detailed Equipment, Manufacturer, Serial Number, Model, and locations of equipments within NIH) EVALUATION: In addendum to 52.212-2; the following factors shall be used to evaluate offers. A. TECHNICAL APPROACH (25% ) 1) Describe your firm's technical approach for conducting the preventive maintenance and unscheduled service. List all services that will be provided for conducting the preventive maintenance (quarterly), unscheduled service and repairs, including any exclusions that may arise. Provide a schedule for preventive maintenance that demonstrates an understanding of the scope, critical nature and complexity of the Sterilizer Units. Provide an example of documentation of maintenance evaluations of similar or like systems. Provide an example of documentation that will be provided for unscheduled repair service visits. SCORE /10 2) Submit evidence of experience and qualifications in the firm of: a. knowledge of the types of Sterilizer equipment and parts listed in the SOW; b. manufacturer-trained personnel; c. technical understanding of the maintenance requirements according to manufacturer's specifications of each of these types of equipment; d. technical understanding of problem solving equipment failures and malfunctions; e. ability of the organization to meet the time lines for service and emergency calls; f. quality control and quality assurance methods; and g. access to Original Manufacturer's (OEM) parts during the term of this contract. SCORE /15 B. QUALIFICATION OF PERSONNEL (25%) Provide resumes of available and qualified personnel within company management, as well as technicians who will provide maintenance and repair services. Technicians must have adequate training credentials, including manufacturer training and experience to indicate meeting SOW requirements. SCORE /25 C. PAST PERFORMANCE (20%) Capabilities and Resources of the Company, Past Performance, Level of risk a. The Government will assess the relative risks associated with each offeror. Performance risks are those associated with an offeror's likelihood of success in performing the acquisition requirements as indicated by that offeror's record of past performance. The assessment of performance risk is not intended to be the product of a mechanical or mathematical analysis of an offeror's performance on a list of contracts but rather the product of subjective judgment by the Government after it considers all available and relevant information. b. When assessing performance risks, the Government will focus on the past performance of the offeror as it relates to all acquisition requirements such as cost, schedule, and performance, including standards of good workmanship; the offeror's adherence to contract schedules, including the administrative aspects of performance; the offeror's ability to attract and maintain key personnel minimizing turn-over, the offeror's reputation for reasonable and cooperative behavior and commitment to customer satisfaction and generally the offeror's business-like concern for the interest of the customer. SCORE /20 D. PRICE (30%) Provide a fixed price quote in accordance with the Statement of Work identified. SCORE /30 The price quoted will be evaluated taking into consideration any price reductions. A best value analysis will be performed taking into consideration the results of the technical evaluation, price evaluation, and the ability to complete the work within the Government's required schedule. This solicitation incorporates one or more solicitation provisions by reference with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. (End of Provision) In addendum to FAR 52.212-1; Provide documentation to support that the organization has met these service requirements with similar projects of this size, scope and magnitude. Submit evidence of experience and qualifications of the firm providing comparable preventative maintenance and repairs of sterilizer units of the same vintage/manufacturer/type, as required in accordance with the proposed SOW specifications. Submit names and locations of hospital, and list names and telephone numbers of individuals representing those hospitals, who may be contacted as references (minimum of three) who can speak to the company's quality of products and services, ability to control costs, timeliness of performance and quality of business relations. The Government reserves the right to make an award to that offeror whose quotation provides the best overall value to the Government. Interested offerors may submit proposals electronically to this notice no later than 03/3/2013 Eastern Standard Time 5:00 p.m. Offerors may submit a completed FAR 52.212-3, or cite their Representations and Certifications on sam.gov. The solicitation shall include pricing for the base year and options priced separately. The Government intends to make a single award based on best value to the Government, technical and price factors considered. The anticipated award date for this requirement shall be on or before April 1, 2013.
