- "SOURCES SOUGHT. This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this sources sought synopsis announcement is to gain knowledge of potential qualified sources such as hubzone, 8(a), small disadvantage, service disabled veteran owned business and small business relative to NAICS 621511, size standard of $30 million. Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Vendors interested in providing an offer will have to respond to a separate solicitation announcement that shall be published separately from this announcement. "The Veterans Affairs (VA), Oklahoma City Medical Center is seeking sources for a potential contract to provide Pathology and Laboratory Services to provide specialized (primarily polymerase chain -PCR) reference testing. "Test Menu to include: "Ordering code (contractor's identification code) "CPT Code "Test Methodology "Specimen types "Specimen collection and handling requirements "Test results interpretation or interpretative remarks, if appropriate "Maximum Turn-Around-Time (TAT), excluding time required for repeat assay. "Assay schedule ('DAILY', MWF, etc.) "Unit price and discount, if any, offer to the Government General Qualifications Licensing and Accreditation 1. The Contractor shall have all licenses, permits accreditation and certificates required by law. The reference laboratory must be licensed and accredited by the College of American Pathologists and/or other state regulatory agencies and mandated by federal and state statutes. In addition, the laboratory must be certified as meeting the requirements of the Department of Health and Human Services Clinical Laboratory Improvement Act of 1988. 2. The contractor shall provide a copy of all relevant permits/licenses and certifications inclusive of any sanctions current or pending throughout the USA, prior to contract to contract award. 3. Contractor policies and procedures shall comply with Health Insurance Portability and Accountability Act (HIPPA). Contractor Personnel 1. The contractor shall maintain current accreditation and notify the Contracting Officer Technical Representative (COTR) of any lapse in state license, CLIA certification, or clinical pathology certification. The Contractor shall provide a copy of the renewed licenses/certificates to the COTR before expiration. Test Result Reporting **** 1. Contractor shall provide electronic transmissions (FAX) of all completed and/or partial test results to the laboratory within published Turnaround Time (next day) **** VERY IMPORTANT PART OF THE REQUIREMENT***** The intended contract shall be for a base year, with the Governments' option to extend the contract for one year increments up to four years. Response to this sources sought must include information on how you can meet the above requirements along with the following : 1. Point of Contact, 2. Company Name, 3. Address, 4. Telephone and facsimile numbers, 5. E-mail address, 6. List your Accreditation Status. 7. Describe the services of your company Website (i.e., web site address, discuss client access to view secure information online, download procedures, document retrieval, etc..). 8. Provide overview of provided specialized (primarily polymerase chain -PCR) reference testing provided by your organization. 9. Type of Business such as 8(a), hubzone, small business, large business etc. IF YOU CANNOT MEET ALL OF THE REQUIREMENTS ABOVE PLEASE DO NOT RESPOND TO THIS SOURCES SOUGHT NOTICE. Responses to this notice shall be mailed or faxed or e-mailed to the attention of; Leigh Ann Nunn, Oklahoma City Department of Veterans Affairs Medical Center, 921 N.E. 13TH Street, Oklahoma City, OK 73104, fax number (405) 456-5123 or leigh.nunn2@va.gov. Telephone responses shall not be accepted. Responses must be received no later than 4:00 PM, March 1st, 2013. This notice is to assist the VA in sources sought only. A solicitation is not currently available therefore, DO NOT REQUEST A COPY OF THE SOLICITATION. If a solicitation is issued it will be announced at a later date, and all interested parties must respond to that solicitation announcement separately from the responses to this announcement. Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. Again, at this time, no solicitation exists.
