Description

AMENDMENT A00001 TO CHANGE THE SITE VISIT TIME FOR KERRYVILLE NATIONAL CEMETERY FROM 11:30 AM TO 1:00 PM NOTICE: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The Request for Quotation (RFQ) is VA786-13-Q-0177. No other solicitation will be issued in response to this announcement. This announcement will serve as both the synopsis and solicitation. Quotes in response to this combined synopsis/solicitation are due March 25, 2013 by 2:00PM MST for both cemeteries and shall be emailed to Geraldine.Herrera@va.gov. Purchase order award will be made to the quoter/offeror whose quote is responsive and offers the lowest aggregate price to perform the minimum quantities as specified in this announcement. Past performance when combined with price is significantly more important. The offerer shall provide a list of references completed within the last 3 years similar in size and scope and complexity. The Government reserves the right to award to other than the lowest price quoter. This RFQ document incorporates provisions and clauses which are in effect through Federal Acquisition Circular FAC 2005-61. The type of purchase order being contemplated is indefinite quantity type, and the actual number of trees will be determined based upon funding available at time of award. The associated North American Industrial Classification System (NAICS) code for this procurement is 561730 with a small business size standard of $7.0M. This requirement is set-aside 100% for Service Disabled Veteran Owned (SDVOSB) Small Business Concerns; and only qualified and eligible SDVOSB concerns may be considered for possible purchase order award. The period of performance is seven (7) calendar days from receipt of a purchase order award. The place of performance where all work will be accomplished are two sites: Kerryville National Cemetery, located @ Spur Route 100, Kerryville, TX 78028 (two miles Southeast of Kerryville, from Hwy 27, north on Spur route 100, ½ mile to cemetery and Fort Sam Houston National Cemetery, located @1520 Harry Wurzbach Road, San Antonio, TX 78209. Only one organized site visit will be conducted. The contractor shall provide both a unit price and total amount for each of the following Contract Line Item Numbers (CLINS), and all pricing shall be based on the attached statement of work. The contractor shall provide all plant, labor, tools, parts, equipment, transportation and incidentals needed to perform work as shown on the attached Bid Schedule. Questions regarding this announcement shall be emailed to Geraldine.herrera@va.gov. The organized site visit will be March 14, 2013 for both cemeteries,10:00am for Fort Sam Houston National Cemetery and 1:00PM for Kerryville National Cemetery. The deadline for receipt of questions for both cemeteries is March 18, 2013, 2:00PM MST. A SF 1449 is not required and you may submit your bid on your company letterhead. Questions can be emailed to geraldine.herrera@va.gov. Quoters must be registered in the System for Award Management (SAM) at https://www.sam.gov to be eligible to receive purchase order award. In addition, SDVOSB concerns must be registered and have an active account in www.vetbizregitery.gov to be eligible to receive an award. The selected Offeror must comply with the following commercial services terms and conditions, which incorporates by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5- Contract Terms and Conditions Required to Implement Statues or Executive Orders - Commercial Items. The following clauses are included: 52.222-50, 52.219-27, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.232-30, 52.232-34, 52.222-41, and 52.222-26. In addition the following clauses are hereby incorporated as an addendum to FAR Clause 52.212-5.
