Description

This is an amendment to the previous notice of combined synopsis solicitation notice. This notice serves as the pre-solicitation notice. The Department of Veterans Affairs has the need for Home Health Aide and Homemaker Services for beneficiaries of the VA Medical Center, Beckley, West Virginia. Normal working hours for services shall be provided Monday through Friday from 6:30 a.m. until 6:30 p.m. This requirement will be set aside 100% for Service Disabled Veteran Owned Small Businesses (SDVOSB). The NAICS code is 621610. Size standard $13.5M. Contract base period will be from Date of Award to September 30, 2013 with the possibility of four (4) one (1) year option periods, to be exercised at the discretion of the Government. Solicitation documents will be available on or about February 28, 2013 at www.fbo.gov with proposals due on April 1, 2013@ 4:00 pm (eastern standard time) to the following mailing address: Department of Veterans Affairs, Attn: Barbara J. Hawk, VISN 6 Centralized Acquisition Service (90C), Bldg 27 Averill Dr., Hampton, Virginia, 23667. Hard copies of this solicitation must be requested in writing by sending an e-mail to Barbara.hawk@va.gov or faxing your request to (757) 728-3132, Attn: Barbara J. Hawk. Deadline for questions regarding this requirement is March 12, 2013 at 4:00 p.m. All questions should be faxed or e-mailed to the numbers/addresses contained herein. Past Performance Survey's are due by April 3, 2013 at 4:00 pm. by fax. ELECTRONIC OR FAXED PROPOSALS WILL NOT BE ACCEPTED FOR THIS REQUIREMENT AND NO TELEPHONIC QUESTIONS WILL BE ACCEPTED.
