- REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY THIS IS A SOURCES SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The NCO 19, Rocky Mountain Acquisition Center, 4100 E. Mississippi Ave, Suite 900, Glendale CO is performing market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small or emerging small business firms to set aside a single or multiple award contracts. The Government will use responses to this notice to make an appropriate acquisition decision. To provide STAT and referral lab testing services for thirteen Community Based Outpatient Clinics (CBOC) in Montana. To provide testing of Laboratory samples as needed for emergent care decisions and testing of samples that cannot reach the VA lab in an acceptable time frame. Locations requiring this service are: Anaconda, Billings, Bozeman, Glasgow, Hamilton, Havre, Kalispell, Lewistown, Missoula, Great Falls, Glendive, Anaconda, Miles City, Cut Bank. Testing in Miles City needs to be available 24/7, 7 days a week to serve the needs of our Community Living Center in Miles City. A subset of sites will be asked to provide occasional collection and processing of venous blood samples and urine samples from veteran patients as ordered by VA clinician, Processing and packaging of those specimens according to provided directions and preparing a shipping box for courier pickup (transport charges paid by VA). Locations requiring this additional service are: Bozeman, Glasgow, Hamilton, Havre, Great Falls. High Risk HPV typing of Thin Prep samples as required of samples forwarded from VA Montana Healthcare, Ft. Harrison, MT to a lab in Montana, estimated workload is 30-40 specimens per month. The Contractor(s) shall provide services within the community so as to assure a maximum sample transport time of 30 minutes and a maximum STAT time to result once sample has arrived of one hour. The Contractor(s) shall be approved to provide clinical diagnostic laboratory tests under both Medicare and Medicaid regulations and all applicable federal and state laws. Lab tests needed are urgent or STAT care decisions, Hematology, Urinalysis and Culture & Sensitivity samples, Sedimentation Rate, Blood Gas Analysis, Ammonia levels, Blood, Body Fluid and tissue cultures, etc. Payment will be on a cost per test basis for this requirement. Wilson, Suite 703G, Bozeman, MT 59715; Cut Bank CBOC #8 2nd Ave. SE, Cut Bank, MT 59427; Glasgow CBOC 630 Second Ave South, Suite A, Glasgow, MT 59230; Glendive CBOC 2000 Montana Ave, Glendive, MT 59330; Great Falls CBOC 1417 9th Street South, Suite 200 and 300, Great Falls, MT 59405; Kalispell CBOC 31 Three Mile Dr Ste 102, Kalispell, MT 59901; Lewistown CBOC 629 NE Main Street, Suite 1, Lewistown, MT 59457; Miles City CBOC 210 S Winchester, Miles City, MT 59301; Missoula CBOC 2687 Palmer Street, Suite C, Missoula, MT 59808; Hamilton Clinic 299 Fairgrounds, Suite A, Hamilton, MT 59840; Havre Clinic 130 13th Street, Suite 1, Havre, MT 59501. The information submitted shall contain the company's business size status. This is a request for information and sources only, which may or may not lead to a future solicitation!
