- THIS IS A SOURCE SOUGHT NOTICE ONLY. THIS IS NOT A REQUEST FOR PROPOSAL. The VISN 19 Rocky Mountain Network Contracting Center, 4100 E. Mississippi Ave, Glendale CO is performing market research to determine if there is a sufficient number of qualified (1) Service Disabled Veteran Owned Small Business; (2) Veteran Owned Small Business; or (3) Small or emerging small business firms to set aside a single or multiple award contracts. The Government will use responses to this notice to make to an appropriate acquisition decision. The requirement is for on-site sharpening and repair of all surgical instruments. Services will include but not limited to, sterilization container repair, general sharpening, spring replacement, ophthalmic instruments, arthroscopic instruments, align and sharpen laparoscopic instruments, micro instruments, refurbishing and reinsulating shafts as needed, diamond dusting of forceps and needle holders, repair and sharpening of all minimally invasive instruments, ratchet repair, bipolar repairs, power/ pneumatic, phaco hand pieces, graspers, dissectors, light cables, rigid and flexible scope servicing and repair of all specialized instrumentation and retractors. All repairs must be done on-site with same day turn-around time. For instruments requiring extensive repair and sent to lab services, a seven day time frame is requested. There are two areas that may require this service. The area for this contract shall be, VA Salt Lake City Health Care System, 500 Foothill Drive, Salt Lake City, UT 84148 and Grand Junction VA Medical Center, 2121 North Avenue, Grand Junction, Co 81501. The anticipated period of performance will be one basic year plus three (3) one-year pre-priced options for renewal. All interested firms who can meet the requirements stated above should respond, in writing, with information describing their interest, capabilities, geographic location and indicate which small business category for which they qualify to the Contract Specialist Ronnie Jones no later than 3:00 p.m. (Mountain Time), March 18, 2013 Solicitation VA-259-13-R-0415 -On-Site Instrument sharpening and repair has been assigned and should be referenced on all correspondence regarding this announcement. The NAICS Code 811219 is applicable to this acquisition, the size standard for a firm to be considered has an Emerging Small Business is no greater than 50 percent of NAICS Code with a Business Size Standard of $19.0 M. Interested firms are reminded that in accordance with FAR 4.12, prospective contractors shall complete electronic annual representations and certifications in conjunction with FAR 4.11 required registration in the System for Award Management (SAM) database prior to award of a contract.
