- Provide training and assistance for the Registered Nurses (RN) in the clinic to develop procedures and implement case management. Case management is needed to accurately reflect the condition of our patients who have chronic medical conditions and would benefit from having an RN monitor them. Case Management allows the RN's to assess the current needs and anticipate future needs of the patients and will allow for prevention and education to be provided to the patient and family. The objective is to have all sixteen (16) Registered Nurses at Warm Springs Service Unit, Warm Springs, Oregon, trained in the process and implementation of case management. This will allow for more timely, accurate and cost effective services to be provided to the patients. Case management is a method by which Nursing can assess the medical, social, emotional and economic needs of the patient and make sure that they are addressed. It will allow Providers to know how their patients are doing and if they are being compliant with their treatment program or if changes need to be made. Under tier three in the Model Medical Home designation for which we have applied, we are required to have formal case management services. The case manager will work in conjunction with the health care team which will include Tribal programs as well, to promote an optimal level of wellbeing for the patient. The case manager will be advocates for finding appropriate care for patients needing special services, identify those services and help to implement them for an optimal patient outcome. Case management is essential in helping to prevent return hospitalizations within thirty days which is a CMS goal.
