Description
The Contractor shall furnish a hearse (black panel type truck or van) and related services for transporting deceased personnel (human remains) between the Camp Zama Medical Clinic and military hospitals at Yokosuka Naval Base or Yokota Air Base for autopsy; and transport deceased personnel from other locations in the Honshu area to the Camp Zama Medical Clinic and subsequent transport to one of the hospitals for further disposition in accordance with (IAW) the terms and conditions of the Performance-based Work Statement, Blanket Purchase Agreement (BPA) and applicable Government of Japan (GOJ) laws for land transportation. Performance period start date is 1 April 2013. This will be a pre-priced BPA. The BPA is anticipated to be effective 31 March 2018 unless cancelled earlier by the Contracting Officer. The solicitation is exempted from set-aside for small business for all trucking service. Award will be made to the offeror whose offer represents the best value to the U.S. Government considering the evaluation factors stated in the solicitation. Synopsis of the requirement: Pickup/Delivery: The Government will furnish materials such as a box, bag, plastic or other material to cover or place human remains in before transporting. An U.S. MEDDAC escort personnel or concerned will accompany (ride in) the Contractor's vehicle from the pickup point to Yokosuka Naval Base or Yokota Air Base, if required. The Government will transfer the human remains from the building or other locations to the Contractor's vehicle at the pickup point. The Government will transfer the human remains from the Contractor's vehicle to the building or other locations at the delivery point. If required, the Contractor shall prepare double-bagged ice using the Government-Furnished ice machine and plastic bags as appropriate. The Contractor shall place the double-bagged ice on body at the Contractor's Facility prior to transport to Yokosuka Naval Base, Yokota Air Base or other locations designated by the U.S. Government. Free Time: The Contractor shall provide free time to the Government for thirty (30) minutes for pickup/delivery, which commences when the vehicle is placed by the Contractor at the pickup/delivery points and notice thereof is given to the appropriate personnel (e.g., MEDDAC escort personnel, BPA Caller, Government Representative (GR). All costs of operation under this contract shall be borne by the Contractor, including but not limited to tolls, licenses, costs of overtime, Sunday or holiday work, or for operations during stormy or inclement weather except for those authorized Additional Charges -- Toll Charges (One-way only): The actual costs for tolls will be paid to the Contractor to use toll road(s) to expedite pickup/delivery to the BPA Caller. Midnight and Early Morning Charges: If the service is provided by the Contractor between 2200 hours and 0500 hours, Midnight and Early Morning Charges will be added to the unit price in the Price List. Basic charge will be applied for the first thirty (30) minutes and extended charge will be added to every thirty (30) minutes thereafter. Demurrage Charge: If through no fault of the Contractor, a vehicle is not dispatched or released within the allowable free time, the Contractor shall be entitled to demurrage for detention of the vehicle by the Government beyond the free time at the rate in the Price List. Furnishing Water Protection Sheet: The price for water protection sheets will be charged at the rate in the price list when furnished by the Contractor upon request of the Government. Utility Cost: The fixed price for monthly utility cost for the Ice Making Machine, model IM-35L, 18kg capacity, will be charged at the rate in the price list on a monthly basis. Invoice: An itemized invoice shall be made on monthly basis for all services performed during a billing period, and submitted to the GR by the end of the following month. NOTE: THIS PROJECT WILL BE PERFORMED IN ITS ENTIRETY IN THE COUNTRY OF JAPAN. THE SUCCESSFUL OFFER MUST BE LICENSED AND REGISTERED TO PERFORM THE SERVICE IN THE COUNTRY OF JAPAN.
