Description
Solicitation with attachments (FA2823-13-R-0005) The Presolicitation Notice for this acquisition is waived based on FAR 5.202(a)(13). This notice will serve as the formal solicitation that will only be posted for 30 days. STATEMENT OF WORK FOR RESURFACING FLOOR IN BUILDING 78 96th MXS, EGLIN AIR FORCE BASE, FLORIDA 8 February 2013 SECTION 1.0 SERVICES 1.1. OVERVIEW. 1.1.1. The current poured epoxy-resin flooring installed in 2005 for Eglin AFB Bldg 78, Rooms 101, 101A, 103, 105, 107 and 108 has deteriorated through normal wear and tear and continued operations tempo of a 2-shift operation. Floor resurfacing of approx. 5,899 sq. ft. of high traffic areas are identified by the customer. Resurfacing must meet requirements of manufacturer's recommendations. 1.2. SPECIFIC TASKS. 1.2.1. The contractor shall accomplish applicable tasks in accordance with manufacturer's recommendations. 1.2.2. Service is to be completed by taping off the affected areas, diamond grinding the existing high-traffic areas in multiply rooms (referenced above) without damage to flooring substrate. Blastracing may be required. The Contractor will apply color matching primer and static control epoxy top coating that hardens to a gloss finish, meeting manufacturer's specification. 1.2.3. The contractor shall supply equipment, tools, personnel, training and technical support necessary to resurface 5,899 sq. ft. of existing poured Epoxy-Resin flooring installed in the fall of 2005 effecting high traffic areas in laboratory Rooms 101, 101A, 103, 105, 107 and 108 as required by manufacturer's recommendations. 1.2.3.1. Application of various components of the original flooring system may include application of Stonset PM5 grout or Stonset OP2 osmotic grout (to repair chips caused from normal wear & tear and ensure hydrostatic and or osmotic pressure), Stonlux ATK primer and final Stonkote AT5 (or equivalents), static dissipative coating meeting requirements of the original flooring installation IAW manufacturer's recommendations will complete the resurfacing process. 1.2.3.2. The flooring must withstand cart traffic and hand trucking on plastic/rubber wheels with a maximum load of no more than 1,000 lbs. 1.2.3.3. The contractor shall ensure work, services, and operations are in compliance with local, state, and federal laws and regulations. Additionally, 1.2.3.4. The contractor shall comply with security and environmental requirements applicable to the state of Florida and Eglin Air Force Base. 1.2.4. The contractor will provide the Contract Management Office (CMO) with a list of all materials to be used during the contract period. This list will be provided to the Haz-Mat Cell office on base prior to the start of the contract. Any changes to the materials during the contract period shall be reported to the CMO. The contractor shall not use any materials on Eglin Air Force base until notified by CMO that the materials are allowed to be used on Eglin Air Force Base. 1.3. GENERAL REQUIREMENTS. 1.3.1. The contractor shall provide a minimum of 1 year warranty on materials and labor. Section 2.0 SERVICE SUMMARY PERFORMANCE Pricing Assumed: - Pricing will assume prevailing wage labor. - Customer to provide proper temperature (60F-80F), lights, power (110v). - Contractor will provide a dumpster and provide for all trash removal. - Customer will provide location for dumpster. - Customer will provide a dry, heated laydown area for material storage in advance of work. - Contractor will offload all material into the lay down area provided by the Customer. - Area is to be free and clear of all trades and moveable equipment prior to Contractor's arrival. - Dust control during preparation and application of surface coating is Contractor's responsibility - Area to be free of all traffic and activity during the installation and for a twelve-hour period after it is completed for cure time. - Area must be dry and remain dry and free from drips and leaks during the installation and cure time. - Finished floor will follow contour of existing substrate. - Single source warranty for 1 year covering both materials and workmanship. -- Warranty should include that product will bond to the substrate, will not blister, will be free from manufacturing defects, and will otherwise conform to the Product Data Sheets and Chemical Resistance Guides that were in effect at the time of the commencement of installation. - Testing of the substrate for hydrostatic and/or osmotic pressure prior to installation If it is determined that hydrostatic and/or osmotic pressure exists, Contractor will install Stonfil OP2 (or equivalent) that meets manufacturer's recommendations. - Customer will be responsible for moving all equipment/cabinet on wheels out of the installation areas. - Customer will be responsible for putting plastic over all equipment left in work area - Customer is responsible for post installation floor protection, if required. SECTION 3.0 GENERAL INFORMATION 3.1. Contractor Employees. The selection, assignment, and management of contractor employees is the responsibility of the contractor; however, the contractor shall not employ any individual who is identified as a potential threat to the health, safety, security and general well being, or operation of the installation's mission or personnel. Contractor employees shall present a neat appearance and be easily recognized as contractor employee by displaying the proper security identification. The contractor shall be responsible to ensure that all Federal, State, County licensing and certification requirements are met and kept current. The contractor shall provide documentation of employee licensing and certification upon request. 3.2 CONTRACTING OFFICER. Any matter concerning a change in the scope, price, terms or conditions of the contract shall be referred to the Contracting Officer. 3.3 QUALITY ASSURANCE. The Government will inspect the services provided to ensure compliance with this PWS. The Government will accept the services prior to acceptance of the invoice in Wide Area Workflow (WAWF) by the contractor. 3.4 HOURS OF OPERATION. The terms of the contract will minimize interference with normal daily operations tempo of the Eglin PMEL. All work will be conducted during periods commencing on Friday and ending on Sunday. The hours of operation for this contract shall be between the hours of 6:00 A.M. to 11 P.M. Friday through Sunday. 3.5 Hours of Operation other than Normal. The contractor shall work whatever hours necessary to complete requirements of the contract and resolve any emergency occurring within his or her area of responsibility. 3.6 SECURITY. 3.6.1. Security Requirements. The contractor will provide the CO a list of individuals requiring access to the base. The 96th Contracting Squadron will then notify security forces. Upon arrival to the base, the contractor employee will go to the Visitor Control Center located either at the East or West Gate of Eglin Air Force base to obtain their access card. All contractor employees shall comply with current base access requirement operate their vehicles in accordance with Eglin AFB Base traffic rules and regulations. The Government reserves the right to limit or deny access to the installation as required. 3.6.2. Restricted Areas. The service provider will not be allowed to enter any restricted areas unescorted. 3.6.3. Government Facilities Access. The 96 MXS building custodian(s) will provide access to the areas in Bldg. 78 where the service will be provided based upon the days and times when the access is required on a non-interference basis. 3.7 SAFETY. 3.7.1. Government and Personal Property. The contractor shall conduct work so that property, personnel, and work areas shall be protected at all times from inconvenience, damage, or injury. In the event of damage of any nature caused by contractor operations or employees due to improper protection, precaution, or safety measures, such damages shall be repaired or replaced by the contractor's expense. 3.7.2. Safety Requirements. While performing work under this contract, the contractor shall: 3.8.2.1. Conform to the safety requirements contained in the contract for all activities related to the accomplishment of the work. 3.7.2.1. Take all necessary precautions to reduce the possibility of injury to contractor, government personnel or government equipment. 3.7.2.2. Record and report promptly (within one hour) to the contracting officer or designated program manager, all available factors relating to each instance of damage to Government property or injury to either contractor or Government personnel. 3.7.2.3. In the event of an accident/mishap, take reasonable and prudent action to establish control of the accident/mishap scene, prevent further damage to persons or property, and preserve evidence until released by the accident/mishap investigative authority through the CO. The contractor shall cooperate and assist the Government personnel in the conduct of investigation. 3.8. WORK CLEARANCE REQUEST. 3.8.1. Contractor must obtain and keep a current AF Form 103, Civil Engineering Work Clearance Request, commonly called a dig permit, before any work can begin.
