- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Quotation number W9114F-13-T-0021 is issued as a request for quotation (RFQ). Due Date: 28 March 2013 at 10:00 AM, CEST (local Germany). Quotation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-50, effective 16 March 2011. Not a set-aside. North American Industry Classification System (NAICS) code for this quotation is 423450. The Europe Regional Contracting Office has a requirement to purchase a JBC Corp Demining Medical Support Kits, Casualty Evacuation Sets, Demining Training Supply Kits and all replenishment items tailored to specific customer requirements. Brand Name or Equal. The Government will make an award based on the Lowest Price, Technically Acceptable (LPTA) offer. Quoters shall submit prices on the list of JBC Corp Demining Kits, see below. Para (b) Replace subparagraph (4) with the following: Salient Characteristics and Technical Evaluation Factors: The vendor must explain how they will comply with the requirements in the request for quote by submitting a written quote which describes: JBC Demining Medical Support Kits Salient Characteristics and Technical Evaluation Factors: Technical Evaluation Factors 1) Vendor shall providecomplete Deminer Medical Support Kits, Casualty Evacuation Set, Deminer Training Supply Kits and all replenishment items tailored to specific customer 2) Vendor shall be capable of delivering all items to USAMMCE by the Required Delivery Date of 12 April 2013. 3) Vendor shall be capable shipping door-to-door to Pirmasens, Germany. 4) Vendor shall have a current registration in the System for Award Management (SAM) Salient Characteristics 1) The vendor shall provide DEMINING Kits (JBC PN 50-10) and consumable supply replenishment kits (PMN 50-10R) which will provide supplies necessary to administer immediate emergency trauma care for individual and self aid without medical trained personnel or the need to transport patient. 2) The vendor shall provide MEDIC Sets (JBC PM 50-11) and consumable supply replenishment kits (JBC PN 50-11R) which will provide the supplies necessary to support and administer minor medical care in remote settings. 3) The vendor shall provide CASEVAC Sets (JBC PN 50-12) and consumable supply replenishment kits (JBC PN 50-12R) to facilitate the rescue, recovery, and evacuation of trauma casualties, provide the capability to stabilize and sustain in case of a delayed or difficult casualty evacuation, and adapt to military and non-military modes of transportation. The Trauma Pack in the CASEVAC Set must include a light emitting condenser, flat panel light strip to illuminate the inside of the pack. The inverter power source must be internally mounted in the bag and powered by 3 AA Batteries. The light must be moisture resistant, and remain functional despite damage that may occur to any section of the light strip, including nicks, cuts and slices. The light output from the strip, must not negatively impact night vision. The trauma pack must also include 1 quote mark Raptor side release buckles on the shoulder harness, where it attaches to the pack. The pack must include a mounting platform with two large pouches, which can be removed by quote mark D quote mark rings. The MedSled VLR evacuation sled will be the Vertical Lift Stretcher employed in the CASEVAC Set. 4) The vendor shall provide TRAINING Supply Sets (JBC PN 50-13) and consumable supply replenishment kits (JBC PN 50-13R) which will contain the supplies contained in the DEMINING, MEDIC and CASEVAC sets and the appropriate training models with respective supplies and components for demonstration and training for up to 25 students. 5) Each set will be individually packed in a quote mark JBC Grab and Go quote mark mobile case system; military standard (MIL-C 4150J, MIL-STD-648C, MIL-STD-810F). The cases are 100% waterproof and can be submerged without water penetration. 6) The vendor shall provide Medical Instruction for all Kits and set support required. 7) All sets must be contained and assembled to allow for man portability and multi-user access. 8) The vendor shall provide TCCC guidelines with 9 Line MEDEVAC request, patient card, and Instructional medical literature language translated into Eastern Armenian by a Certified Medical Translator. The translated medical instruction and literature is a necessary component of the set and must be included in the set at the time of delivery (without back order). Failure to receive an acceptable rating of any of the salient characteristics or technical evaluation factors will result in an unacceptable rating unless the error(s), deficiency (ies) or omission (s) can be corrected. All CLINs shall be priced. If you fail to price all Items, your quote will not be considered for award. Para (c) Change 30 to 60. Para (d) Does not apply to this request for quote (RFQ). Para (i) Does not apply to this RFQ. Add the following: Para (m) Quotes shall describe how your offer meets the salient characteristics of this solicitation. Para (n) ELECTRONIC OFFERS. Vendors may submit electronic quotes in response to this RFQ. The quote must arrive by the time specified in the RFQ to the following email address: elena.hughes@us.army.mil Vendors may call Elena Hughes (+49)6371-9464-5434 to verify receipt of the quote. If the vendor chooses to email the quote, the Government will not be responsible for any failure of transmission or receipt of the quote, or any failure of the vendor to verify receipt of the emailed quote. Para (o) QUESTIONS. Prospective vendors are strongly encouraged to submit any questions regarding this RFQ two days before closing date of quote submission by e-mail to Fax (+49)6371-86-8070. All questions received will be answered officially through an amendment for distribution to all prospective vendors. ACQUISITION PROCEDURE This is a commercial item under FAR Part 12 and the acquisition procedures to be used for this purchase will be in accordance with FAR Part 13. ADDENDUM TO 52.212-2 Paragraph (a) is hereby replaced with the following: (a) The Government will award a purchase order to the responsible quoter whose quote represents the best value to the Government, price and other factors considered. Award will be made on the basis of the Lowest Price, Technically Acceptable (LPTA) quote. If any on salient characteristic below is rated quote mark Unacceptable, quote mark then the Government will no longer consider issuing a purchase order to that quoter, the quoter will be ineligible for award. Evaluation Standards: Evaluation Standards: The Government will use the following standards for a final rating: ACCEPTABLE: A quote that meets the Government's salient characteristics and technical evaluation factors identified in the solicitation. UNACCEPTABLE: A quote that fails to meet the Government's salient characteristics and technical evaluation factors identified in the solicitation. Factor II - Price. The Government will select the quote that is based upon the lowest evaluated price, technically acceptable quote. All CLINS were priced by the Quoter. If the Quoter failed to price all CLINS, the quote will not be considered for award. (b) A written order, mailed or otherwise furnished to the successful quoter within the time for acceptance specified in the quote, shall constitute an offer by the Government to buy the services/supplies called for in this RFQ. A contract will be established when the quoter accepts the Government's offer. (c) The word quote mark quoter quote mark shall be substituted for the word quote mark offeror quote mark wherever it appears in FAR 52.212-3, Alternate I and DFARS 252.212-7000. (End of Provision)
