- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quote (RFQ), solicitation number W912D0-13-T-0002 in accordance with FAR Part 12, quote mark Acquisition of Commercial Items quote mark and FAR Part 13 quote mark Simplified Acquisition Procedures quote mark. If there are areas where FAR Parts 12 and 13 conflict, FAR Part 12 will take precedence. The solicitation document, incorporated provisions, and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-66 dated 28 Feb 2013. This requirement is set aside 100% for Small Business. The North American Industry Classification System code is 541380, Testing Laboratories, with a business size standard of $14M. The 413th Contracting Support Brigade, Regional Contracting Office Fort Wainwright Alaska, is seeking a source to provide all labor, supervision, management, transportation, equipment, supplies, and communication to collect, analyze, and furnish written laboratory reports based on the findings, on an ''as required quote mark basis, for samples (solid, liquid & sludge phases). The term of this contract will include a base year with four potential Option Years. This will be a Firm Fixed Price contract with an anticipated start date for the base year of 1 May 2013. The samples will be drawn from various types of containers and/or soils at Fort Wainwright Alaska & its training areas (Donnelly, Bolio Lakes & Black Rapids) and Seward Army Recreation Center, Seward Alaska utilizing the most current Environmental Protection Agency (EPA) Test Methods for Evaluating Solid Waste Physical Chemical Methods (SW-846), and/or Alaska Department of Environmental Conservation (ADEC) directed test methods. Sources of the samples include but are not limited to drums, cartons, underground storage tanks, oil/water separators, above ground tanks, soil (up to 4 feet deep) or debris piles. A list of the sampling and testing parameters can be found in the Performance Work Statement (PWS) which is attached (Attach 1). It is anticipated that approximately 90% of the samples will be drawn at Fort Wainwright and 10% at the other sites listed above. The approximate round trip distances to the sites are as follows: Donnelly Training Area - 280 mile round trip from Fairbanks, AK Bolio Lakes - 240 mile round trip from Fairbanks, AK Black Rapids - 260 mile round trip from Fairbanks, AK Seward Army Recreation Center - 300 mile round trip from Anchorage, AK DELIVERABLES WITH QUOTE LABORATORIES For each laboratory to be utilized for sample analysis under any resulting contract, the Contractor shall provide documentation addressing each of the items listed in the EPA quote mark Test Methods for Evaluating Solid Waste (SW-846) quote mark paragraph 4.6 Laboratory Operation/Laboratory Records as follows: Operating Procedures Quality Assurance Plans Equipment Maintenance Documentation Proficiency Calibration Records Sample management Original Data Quality Control Data Correspondence Deviations Final Report The submittal shall be a laboratory plan or written document stating how the laboratory will achieve compliance for each item. The submittal shall be part of the technical review process for award. These documents will also become part of the contract and enforceable upon award. Provide any EPA or State quote mark Notice of Violation, Letter of Deficiency, Letter of Warning or similar notification from the EPA or state of deficiencies received by the contractor or the subcontracted laboratory in the past five (5) years. All laboratories utilized under this contract must be certified by the National Environmental Laboratory Accreditation Program (NELAP). To become certified, to renew certification, or to become recertified a laboratory must adhere to the requirements found in Chapter 4, quote mark Accreditation Process quote mark, of the National Environmental Laboratory Accreditation Conference Standards approved in July 2002. PAST PERFORMANCE Please send the attached quote mark Past Performance Questionnaire quote mark (Attach 3) to companies that you have current or past contracts with that are similar to this procurement (within the past 5 years). The companies should forward the questionnaires to this office by the date and time the quotes are due. If you do not have relevant past performance please state so on your quote. QUOTES Quotes shall be submitted using the attached quote excel sheet (Attach 2). Ensure you fill out each of the five tabs representing the base year and each subsequent option year. Failure to complete all of the spreadsheets (5 ea) will render your quote as nonresponsive and the quote will not be considered. The pricing listed for each test on the quote sheet should include all associated costs for the entire process (sampling, testing, travel, reporting etc.). Review the attached quote mark Fast Access to Fort Wainwright quote mark (Attach 4) prior to preparing your quote. CONTRACTOR MANPOWER REPORTING The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report all contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address https://cmra.army.mil/login.aspx. The required information includes: (1) Contracting Office, Contracting Officers, Contracting Officer's Technical Representative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including subcontractors); (6) Estimated direct labor dollars paid this reporting period (including subcontractors); (7) Total payments (including subcontractors); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each subcontractor if different); (9) Estimated data collection costs; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (11) Locations where contractor and subcontractors perform work (specified by zip code in the United States and nearest city, country, when in an overseas location, using the standardized nomenclature provided on the website); (12) Presence of deployment or contingency contract language; and (13) Number of contractor and subcontractor employees deployed in theater this reporting period (per country. As part of this submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year. Contractors may use a direct XML data transfer to the database server or fill in the file on the website. The XML direct transfer is a format for transferring files from a contractor's systems to the secure website without the need for separate data entries for each required data element at the website. The specific formats for the XML direct transfer may be downloaded from the website. Note: Information from the secure website is considered to be proprietary in nature when the contract number and contractor identity are associated with direct labor hours and direct labor dollars. At no time will any data be released to the public with the contractor name and contract number associated with the data. On the quote spreadsheet (Attach 2) there is a block labeled CMR (Contract Manpower Reporting). If you intend to charge the government for the time needed for the reports, please indicate the amount on each of the spreadsheets. If you do not intend to charge for completing the report, enter a quote mark 0 quote mark on each of the spreadsheets. QUALITY CONTROL PLAN A quality control plan must be included with the submission of the quote. PROVISIONS: The provisions at 52.212-1, FAR 52.212-3, quote mark Offeror Representations and Certifications-Commercial Items quote mark applies to this acquisition and offeror(s) must include a completed copy with its offer. FAR 52.212-3 can be obtained from http://farsite.hill.af.mil. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at the System for Award Management (SAM) site. If an offeror has not completed the annual representations and certifications at the SAM website, the offeror shall complete paragraphs (c) through (o) of this provision. EVALUATION PROCEDURES: The Government intends to make a single award to the responsible contractor who has the best value Lowest Price Technically Acceptable (LPTA). The award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors. Past performance will be reviewed IAW FAR 9.105-2(a)(1). The quote must be in full compliance to all other requirements set forth in the solicitation and PWS. As part of the price evaluation, the Government will evaluate the Option to Extend Services under FAR Clause 52.217-8 by adding one-half of the offeror's total final option period price to the offeror's total price. Thus the offeror's total price for the purpose of evaluation will include the base period, first option, second option, third option, fourth option, and one-half of the fourth option. Offerors are required only to price the base and the four options. Offerors SHALL NOT submit a price for the potential six month extension of services. FAR CLAUSES: The clause at 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition. 52.212-5, Contract Terms and Conditions Required to Implement Statutes or executive Orders-Commercial Items is applicable as are the following clauses within 52.212-5; 52.222-50, 52.233-3, 52.233-6, 52.204-10, 52.209-6, 52.219-8, 52.219-13, 52.219-14, 52.219-28, 52.222-3, 52.222-17, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-40, 52.222-41, 52.222-42, 52.222-43, 52.222-54, 52.223-18. In addition the following FAR clauses are applicable: 52.203-3, 52.216-18, 52.216-21, 52.217-5, 52.217-8, and 52.217-9. DFAR CLAUSES: DFAR 252.212-7001 is applicable to include the following clauses: 252.203-7003, 252.232-7003, and 252.246-7002. Only those offerors determined to be in compliance with the requirements as stated in this solicitation will be considered for award. If additional information is required to support price reasonableness, each offeror may be required to provide additional information as needed. The government reserves the right to use price realism analysis in the quote review process to determine if any pricing is unrealistically low and reflects a lack of understanding of the technical requirements or indicates a high risk of failure. No partial quotes or quotes without specifications will be accepted. Non-conformance with this requirement may result in the offerors quote being determined unacceptable. To be eligible for an award, contractor must be registered in the System for Award Management (SAM) database. NO EXCEPTIONS. A contractor can contact SAM by calling 1-866-606-8220 or e-mail at www.fsd.gov. A DUNS (Dun and Bradstreet) number is required in order to register. All invoices shall be submitted through Wide Area Work Flow. You can register at https://wawf.ef.mil/index.html.
