- Questions and Answers (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a separate written solicitation will not be issued. (ii) Solicitation number 13-236-SOL-00017 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-64. (iv) The North American Industry Classification System (NAICS) code is 541910 with a size standard of $14M. The Indian Health Service (IHS), Division of Acquisition Policy intends to solicit a firm fixed price order to Small Businesses only. (v) The Contract Line Items (CLINs) and pricing structure are as shown below. CLIN 0001 -- Contractor shall provide all deliverables for the objectives listed in the attached statement of work. - Total$_____________ CLIN 0002 (Optional Line Item) -- Travel Cost for Object 4 (See SOW Section 2.4) - Total$___________ CLIN 0003 (Optional Line Item) -- Travel Cost for Object 5 (See SOW Section 2.5) - Total$___________ ** Travel within CONUS may be required. CLINS 0002 & 0003 will be reimbursed ONLY if on-site presentation is required. On-site presentation may change to virtual delivery. (vi) The IHS is in need of a subject matter expert to analyze hospital accreditation/certification survey information, identifying the findings common across the system of hospitals as well as those unique to individual hospitals. This will support the spread and development of solutions addressing common weaknesses. See the attached for the detailed scope of work. (vii) The delivery and acceptance term for this order is FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial Items (Feb 2012), applies to this acquisition and a statement regarding any addenda to the provision (ix) 52.212-2, Evaluation - Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government intends to evaluate offerors and award a contract without discussions with offerors. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if later it is determined by the Contracting Officer to be necessary. The Government may reject any or all offers, waive informalities and minor irregularities in offers received. The following factors shall be used to evaluate offers: The acceptability of the Technical Quotation and Price will be evaluated. **See the attached Statement of Work Section 2.12 -- Source Selection Criteria, for technical evaluation factors. The result of the technical evaluation is more important than the pricing evaluation.
