Attachment A - List of States and Cities 1. Introduction This is a Small Business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small businesses; HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. The Applicable NAICS code for this acquisition is 621999 - All Other Miscellaneous Ambulatory Health Care Services 2. Background The purpose of this acquisition is to provide FOH with a network of medical providers of occupational health medical services nationwide. Physician(s) may need to be board certified in any of the following: cardiology, emergency medicine, internal medicine, family medicine, or preventive medicine (aerospace, occupational, or general preventive medicine and public health). Other medical professional, i.e. optometrist, nurse practitioner, physician assistants and nurses shall have a current and unrestricted license to practice medicine in the jurisdiction of delivery of care. Staff certifications for Hearing Conservation by a Council for Accreditation in Occupational Hearing Conservation (CAOHC) approved program are required for technicians conducting audiometric testing UNLESS an audiometric microprocessor is used. The goal is to provide consistent standardized occupational health services in areas where FOH does not have a current presence in an efficient manner such that Federal employees do not have to travel long distances to receive services. Physical examinations will be completed by a health care provider whose knowledge, skills, and abilities are commensurate with the scope of practice per the state or territory requirements in which the service is performed. The contractor will be responsible for managing all aspects of the private provider network, including maintenance of credentialing files. The PPN service requests will include medical surveillance exams, applicant exams, post-exposure exams, phlebotomy services, commercial drivers license exams, respiratory medical clearance exams, diver exams, federal aviation administration exams, department of state exams, hazardous material response unit exams, GYN exams, independent medical evaluations, fitness for duty examinations, immunizations, annual medical screenings, collection of various laboratory specimens, travel medications, and a variety of additional health and wellness procedures and examinations listed under types of services of this notice. Workload capacity could include more than 8,000 exams and over 65,000 Occupational Health Services. 3. Project Requirements The contractor shall provide the network of medical providers within the Continental United States (CONUS), as well as outside of the Continental United States (OCONUS), to include the Virgin Islands, Guam, Puerto Rico, Hawaii and Alaska, and international locations including Canada. Additional international sites may also be required, if the need arises. These service provisions are to be performed through a variety of health and wellness service facilities. Hospitals, occupational health clinics, general medical clinics, specialty and other types of medical providers may be required. Typically around 2000 sites have been used each year, but as many as 5000 during peak utilization. Outreach services, which are those clinical services that are provided at the employee's worksite or other agency requested location. Typical outreach services may include mass immunizations and screenings, and may utilize mobile van services. The contractor shall distribute the necessary forms, documents, and supplies to all medical providers, as specified by FOH, to successfully perform the required services. The contractor shall receive and process the completed clinical documentation from the medical providers and shall review all materials for completeness, adequacy of testing and accuracy according to components and instructions as specified in FOH Clinical Policies and Procedures, and other documents. All testing and services must meet United States occupational health industry standards. The contractor shall administer and manage the private provider network. This requirement includes the location and procurement of services from a network of medical providers in designated locations who meet FOH Occupational Health services requirements. The contractor will also receive documents specific to each examination type and requirements, review them, and schedule services as identified in those documents at clinics able to provide the requested Occupational Health services. The contractor will also prepare and ship laboratory kits and examination packets to the private provider's location. The kits will include instructions on laboratory testing and examination and testing requirements, including the requirements of specified form completion. The contractor will also prepare and ship vaccines, if indicated. Shipment of vaccine should meet or exceed CDC requirements for vaccine shipment. The contractor will monitor and manage the private clinic performance. If the clinic is not able to complete the requirements of the exam type request adequately, the contractor may be requested by FOH to complete a corrective action plan and present it to the COR. The contractor will also maintain and ship medical records to specified locations as identified in the work order or recipient plan. The contractor shall provide a single toll-free number to be distributed to FOH staff. The toll-free number must be staffed from 7:00AM to 7:00PM Central Standard Time. Response Information In order to respond to this notice, interested firms should address the depth and breadth of corporate and technical capabilities, and must be able to clearly convey its experience and/or ability to provide the services in the following areas, which are identified as core services. Types of Services: Screening Exams: brief, intermediate, comprehensive, and wellness Tests that may be required to complete an exam: Audiometry, commercial driver's license, respirator medical clearance, cardiovascular stress test, stress test, stress echocardiography, chest x-ray (PA&Lat) or (PA), colorectal cancer screening (EZ or hemoccult), TB skin test, EKG, spirometry, comprehensive vision (includes near, far, depth, color, fields, lateral/vertical phoria, monocular, and binocular), with titmus or sight screener, examine with and without glasses or contacts (if used). May be required to do additional near vision testing; jaeger test cards or rosenbaum pocket chart, if requested. Additional snellen vision chart test may be required), Best Vision (includes near, far, depth, color, fields, lateral/vertical phoria, monocular, and binocular, with titmus or sight screener, corrected vision only. Additional snellen vision test chart may be required), Ishihara (14 or 24 plate), and tonometry. Specialty Pulmonary Testing: monoxide diffusing capacity, pre and post bronchodilator, bronchospasm evaluation, chest CT scan without contrast, pulmonary compliance study, and pulmonary stress test Specialty ENT Testing: ENT exam, radiological view of sinuses, and CT maxilla facial area Audiometric testing: audiometry with CAOHC certified technician, comprehensive audiogram (audiologist: threshold evaluation, pure tone air and bone and speech recognition, includes full audiogram examination.), hearing aid (binaural and monaural), and pure tone audiogram, and speech audiometry. Cardiology Tests: doppler, color doppler, myocardial perfusion, myocardial perfusion with injection fracture, echocardiography, and monoxide diffusing capacity Specialized Testing: bone density and NMP22 bladder check Additional Occupational Health Services: EKG over reads (stat and routine), and B-reads, collection of various laboratory speciments and Interferon Gamma Release Assays (IGRA). Vaccines: Hep A/B Combination, Hep A, Hep B, influenza, japanese encephalitis, MMR, meningococcal, pneumococcal, polio, rabies imovax or rabavert, rabies booster, TDAP, tetanus, typhoid (injectable and oral), varicella, and yellow fever. Contractor must also have the ability to store and distribute government provided vaccines. Outreach Services: Those clinical services that are provided at the employee's worksite or other agency requested location. Typical services may include mass immunizations and screenings, and may utilize mobile van services. May require use of a mobile vans and outreach medical staff. Outreach services may include blood pressure checks, immunizations, phlebotomy services, audiometry, mammogram, bone density and physical examinations. Travel Medications: There may be a need to dispense travel medications, including Acetazolamide, Chloroquine, Loperamide, Mefloquine, and Doxycycline. Doxycycline and Malarone are supplied by FOH. The private provider will be required to distribute these travel medications when needed. The other required travel medications (Acetazolamide, Cloroquine, Loperamide, and Mefloquine) will be obtained through the private provider clinics by MD prescription. Independent Medical Examinations (IME): The provider requirements are for Board Certified physicians. Independent Record Reviews may be required. Specialties include allergist, cardiologist, audiologist, dermatologist, endocrinologist, gastroenterologist, family practice, hematologist, infectious disease, internal medicine, nephrologist, neurologist, neurosurgery, oncologist, ophthalmologist, orthopedist, otolaryngology (E.N.T.), physical medicine & rehabilitation, psychiatrist, psychologist, pulmonologist, rheumatologist, and urologist. These specialists may be required to testify during court proceedings. Medical Examiners performing CDL's: Medical examiners who perform DOT driver medical exams are expected to understand fully the medical standards of the Federal Motor Carrier Safety Regulations (FMCSRs) and related guidance. The National Registry requires medical examiners to participate in required training, pass a certification test, and register on the National Registry system to become a certified medical examiner. Certified medical examiners agree to keep their National Registry accounts up to date (licenses, training records, etc.) and to transmit at least monthly results of all CMV driver exams performed to FMCSA via the National Registry system. Services listed above are currently being performed in the states and cities listed in Attachment A. An approximate percentage of services utilized per location in fiscal year 2012 are outlined. The locations and services may increase or decrease due to changes in Federal Occupational Health's (FOH's) customer agency needs. List the states and cities where you have the capability to provide all services in this sources sought. Electronic Medical Records. Electronic Medical Records may be prohibited by some Government agencies and therefore will need to be maintained on paper. In those cases, all records must be maintained in a secure, locked filing cabinet or locked room, separate from other medical files and with access restricted to individuals with a need to know and authorized as noted above in accordance with federal medical confidentiality laws. Security Checks: There may be a requirement for the medical providers to complete a background investigation. Only those providers cleared by the government agency will be authorized to perform services for certain federal agencies. This will require the provider and other office staff to complete a security process by completing necessary forms and being finger printed prior to performing exams. Data Systems: NIST Compliance: The contractor will maintain the responsibility of providing electronic management of the data, related to the accomplishment of the work. The contractor shall ensure that the security for the systems supported is compliant with the protocols defined in NIST Special Publication 800.53 rev 3 or updated revisions. The National Institute of Standards and Technology (NIST) Special Publications (SP) 800-53 Rev. 3, NIST SP 800-53A Rev. 1, and NIST 800-37 Rev. 1 establishes guidance and requirements for conducting security authorizations and introduces the concept of a Risk Management Framework (RMF). NIST 800.53 security standards can be accessed at http://csrc.nist.gov/groups/SMA/fisma/assessment.html or http://csrc.nist.gov/publications/nistpubs/800-53A-rev1/sp800-53A-rev1-final.pdf. System owners are responsible for addressing NIST SP 800-53 Rev. 3 in their implementations and for developing the security artifacts that address NIST SP 800-53 Rev. 3 controls. Authorization and security assessment efforts shall be mapped to NIST SP 800-53 Rev. 3 and reflect what controls guidance are implemented in a system. Authorization to Operate (ATO) decisions will also follow these standards with approval decisions based on compliance with NIST SP 800-53 Rev. 3 control requirements. The contractor will be responsible for managing the security of their system in a manner that will detect security breaches. In the event of a security breach, the contractor shall be liable for the Government's costs to notify and/or remediate the breach of private personal data with FOH customers. 4. Anticipated Period of Performance: The Government anticipates issuing an Indefinite Delivery Indefinite Quantity (IDIQ) for a period of five years. Individual task orders would not exceed 12 months. 5. Information Sought Interested sources possessing the capabilities to meet the above requirements should submit a cover letter along with a capability statement not to exceed fifteen (15) pages demonstrating ability to perform and should include an outline of previous or similar projects so that the Government can perform a proper evaluation of the company's capability. The statement should also include an indication of certified small business status. This indication should be clearly marked on the first page of the capability statement. Interested small business firms are highly encouraged to respond to this notice. However, firms should understand that generic capability statements are not sufficient for effective evaluation of their capacity and capability to perform the work required. Responses must directly demonstrate the company's capability, experience, and ability to marshal resources to effectively and efficiently perform the objectives described above. Interested respondents shall provide a capability statement to include the following: Information regarding the difficulty and/or feasibility of the potential requirement or proposed acquisition Information regarding core competencies: (a) Staff expertise, experience, and formal and other training; (b) Current corporate capability and capacity to perform the work; (c) Prior completed projects of similar nature; (d) Corporate experience and management capability; (e) Examples of prior completed Government contracts, references, and other related information; 6. Submission Instructions One copy of response must be submitted not later than 9:00A.M., April 19, 2013 electronically in Microsoft WORD or Adobe PDF format to Madilyn Gonzalez at Madilyn.Gonzalez@psc.hhs.gov. Capability statements will not be returned and will not be accepted after the due date. The maximum number of pages for submission is 15 pages. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality.
