- State of Idaho Worker's Compensation Form Solicitation Package for Mobile Hearing Test, Nez Perce-Clearwater National Forest - return SF 1449, pages 3-5, pages 35-50, and State of Idaho Worker's Compensation Form for your quote This contract requires OSHA Standards Mobile Hearing Tests and related work in compliance with its terms, specifications, and provisions. This includes furnishing labor, equipment, supervision, transportation, operating supplies, and incidentals. LOCATION AND DESCRIPTION The annual mobile hearing tests will be for employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels as required by 29 CFR 1910.95(g)(1) and non-required Forest Service employees. The Hearing Test will be given in six central Idaho locations: Potlatch, Idaho is located 50 miles north of Lewiston, ID on US 95N; 78 miles south of Spokane, WA on US 95S; and 347 north of Boise, ID on US 95N. An estimate of 15 to 35 employees will be tested at the Palouse Ranger District in Potlatch, ID. Orofino, Idaho is located 43 miles west of Lewiston, ID on US 12; 105 miles south of Spokane, WA on US 195S; and 285 miles north of Boise, ID on US 95N. An estimated of 60 to 80 employees will be tested at the Clearwater National Forest Supervisor's Office in Orofino, ID. Powell, Idaho is located 168 miles east of Lewiston, ID on Highway 12; 60 miles southwest of Missoula, MT on Highway 12; and 348 miles north of Boise, ID. An estimate of 15 to 30 employees will be tested at the Powell Ranger Station. Kamiah, Idaho is located 67 miles east of Lewiston, ID on Highway 12; 152 miles southwest of Missoula, MT on Highway 12; and 261 miles north of Boise, ID. An estimate of 60 to 100 employees will be tested at the Kamiah Office in Kamiah, ID. Grangeville, Idaho is located 73 miles southeast of Lewiston, ID on Highway 95; 170 miles southwest of Missoula, MT on Highway 13; and 229 miles north of Boise ID, on Highway 95. An estimate of 60 to 100 employees will be tested at the Nez Perce National Forest Supervisor's Office in Grangeville, ID. Elk City, Idaho is located 51 miles west of Grangeville, ID on Highway 14 and 65 miles south east of Kooskia, ID on Highway 14. An estimate of 25 to 35 employees will be tested at the Red River Ranger Station, Elk City Ranger District in Elk City, ID. The test will be given the first two weeks of June 10 through June 21, 2013. The option years will be tested June, 2014 and June, 2015, actual dates to be determined on renewal.
