Description

This is a combined synopsis/solicitation for commercial Items/services prepared in accordance with the format Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and only an electronic solicitation will be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-64(effective 1/18/2013). This procurement is 100% Total Set Aside for Small Business. The North American Industry Classification Code is 562211 and Size Standard is $35.5 million. Request for Proposal (RFP) number is VA248-13-R-0973. The Government contemplates award of a single contract resulting from this solicitation. Anticipated period of performance is a base year (6) months period and two (2) one-year option periods. This procurement is to furnish all necessary supervision, labor, supplies, equipment, material and transportation to pick up, package, transport, eradicate and disposal/destruction of bio-hazardous waste, pathological waste, and/or special wastes at VAMC, 10 Calle Casia, San Juan, Puerto Rico, VA Mayaguez Outpatient Clinic, Mayaguez, PR, VA Ponce Outpatient Clinic, Ponce, PR, Arecibo Community Based Outpatient Clinic, Arecibo, PR, Guayama Community Based Outpatient Clinic, Guayama, PR, Utuado Rural Clinic, Utuado, PR, Ceiba Community Based Outpatient Clinic, Ceiba, PR, Comerio Rural Clinic, Comerio, PR, Vieques Rural Clinic, Vieques, PR and other VA facilities located in Puerto Rico. See the attached solicitation for full details of contract requirements. Anticipated award date: The contract is expected to be awarded on or before April 1, 2013.
