Description
Functional and Performance Specifications X5900 - Radiographic Unit Pueblo Clinic 1.0Functional and Performance Specifications Contractor to provide specialized, expert equipment relocation of the 719553PVA, GE Discovery 650 XR System currently located at the Pueblo VA Outpatient Clinic at 4112 Outlook Blvd, Pueblo Co 81008 to new location at 4776 Eagle Ridge Circle, Pueblo CO 81008. Contractor equipment relocation services include de- installation, reinstallation, calibration, testing and transportation during standard 8:00 - 5:00p business hours. Contractor services will be performed in accordance with applicable GE Healthcare installation guides and project plans. The Department of Veterans Affairs (VA) will prepare the location for the re-installation consistent with GE Healthcare's written specifications including the installation of necessary system cable and assembly of any necessary equipment or hardware not provided by GE Healthcare, with direction from GE Healthcare. The VA will provide an electrician to disconnect and re-connect power to the system in both locations. SCOPE OF WORK oDeinstallation will include a functional check of the system and any appropriate software back-ups prior to removal and all preparation necessary to ready your system for transport by equipment mover. oReinstallation will include the physical installation of the equipment, calibration to system specifications, and HHS testing as necessary to meet FDA and State requirements. oConduct a pre-move site visit and coordination of room preparation with facility contractor oProvide new cabling, rails or other hardware resulting from changes in size and orientation of the new location or changes in required cable lengths oConduct any repair parts and associated labor needed to bring the unit up to a fully operational condition as described in the next paragraph oProvide labor or rigging to physically remove the system from its current location oProvide transportation or labor to deliver the equipment to the new location oProvide Loss, repair or replacement of system or components, including x-ray tubes, due to transportation or storage of equipment oProvide, when applicable, modifications or corrections to the work scope dictated by concealed conditions encountered in the performance of the work and not indicated by the drawings, or specifications. "Provide repair parts and associated labor needed to bring the unit up to a fully operational system during initial functional check or during re-installation included unless otherwise covered by an existing GE service agreement. "Equipment site drawings for the new location will be provided by Contractor "Contractor to provide a specialized GE service personnel for consultation and oversight for entire scope of work including deinstallation, delivery/ transportation, reinstallation, testing, acceptance and certification "Contractor's work, once completed, must be verified and approved by both GE representative (at contractor's expense) and VA BioMedical Engineering Team/ Radiology Service that equipment is in working condition according to manufacturer's technical specifications for full payment of services "Contractor to provide a warranty (no less than 1 year) for equipment operation within manufacturer's specifications "Contractor to provide a medical physicist certification before acceptance is received by the VA See Floor Plans for Installation Service: Floor plans - X5900 - Radiographic Unit - PUE1 Floor Plans - X5900 - Radiographic Unit - PUE3 Floor Plans - X5900 - Radiographic Unit -PUE2
