Description
EEO AA Non Segregated Davis Bacon REQUEST FOR PROPOSAL Pest Control Services The Brooklyn Job Corps Academy is soliciting bids for the provision of pest control services at their facility, which is located at 585 DeKalb Avenue, Brooklyn, New York 11205. The Brooklyn Job Corps Academy is a federally funded training program for youth between the ages of 16 and 24. The program operates year round with approximately 210 trainees. The Academy is operated by ResCare, Inc. for the U.S. Department of Labor. Work shall be accomplished in accordance with the specifications outlined below. SCOPE OF WORK: • Weekly services of two kitchens and dining areas. • Bi-weekly services of exterior grounds for rodents. • Monthly services of all storage areas, culinary arts Classroom, requested areas in the Medical Unit, Administrative Offices and classrooms. • Apply integrated pest management services (IPM), which will eventually lead to the Academy being pest free. Provisions of services shall be administered in such a way that the vendor warranties the pesticides and/or materials being utilized will accomplish results in treated areas. Vendor should provide upon request a report outlining any areas that may be out of compliance with sanitation practices and/or make recommendations to eliminate any pest-conductive conditions which may exist. PERFORMANCE PERIOD: This agreement shall be in effect from April 1, 2013 to March 31, 2014. Successive renewal of this contract may be made subsequent to annual solicitation of services following the first year. Additional work may be requested from time to time, which shall be authorized by written change to the resultant agreement. SAFETY AND HEALTH: All work shall be accomplished in a safe and efficient manner, with minimal disruption to the operation of the Academy. The subcontractor represents, by acceptance of any resultant agreement that they are in compliance with all regulations and standards flowing from the Occupational Safety and Health Act of 1970 (P.O. 91-596, 29 USC 651 et seq.) and further that in the event hazardous materials are involved, will comply with Federal Standards No. 310 dated July 1971 to the latest change, entitled "Symbols for Packages and Containers for Hazardous Chemical and Materials." Vendor acknowledges that by acceptance of any resultant agreement, that all pest and rodent control applications meet federal, state, local, and OSHA regulations. Vendor shall be required to submit Material Safety Data Sheets (MSDS) for all products being proposed for use under the resultant agreement. PROPOSAL SUBMISSION: Interested parties should respond to the above solicitation by COB on March 28, 2013 to be eligible for award. Proposals should be submitted with the following breakdowns: • Interior areas identified above for service once per month. • Weekly service of Kitchen and Dining areas. • Bi-weekly service for exterior grounds rodent control. • Total minimum for monthly maintenance service rates. • Total minimum for annual maintenance service rate. Bidders are encouraged to schedule a visit to the site and become familiar with the physical layout of the facility where the services are to be provided.
