Description

Brand Name Justification Product Description and Clauses The Department of Health and Human Services is issuing this competitive RFQ for the purpose of entering into one firm fixed price contract. This requirement is for the Department of Health and Human Services Agency for Healthcare and Research Quality (DHHS/AHRQ). See attached Product Description and Brand Name Justification. This RFQ will be evaluated on the basis of Lowest Price Technically Acceptable. Questions must be submitted by March 29, 2013. Interested offerors must submit a response no later than 12:00pm EST on April 2, 2013 electronically to Lisa.Slavin@PSC.HHS.GOV. Each response should include the following business information: 1) DUNS 2) Company name 3) Company address 4) Type of company (i.e., small business, woman owned, veteran owned, etc.), as validated through SAM (System for Award Management).
