- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is solicitation W91YU0-12-T-0066, issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions/clauses are those in effect through Federal Acquisition Circular 2005-66. The associated North American Industrial Classification System (NAICS) Code for this procurement is 541380 (Testing Laboratories), Size standard is $14.0M. This RFQ is being issued as Firm Fixed Price, 100% Small-Business Set Aside. All responsible sources may submit a quotation, which will be considered by the agency. Only a single award will be processed from this RFQ. Non-Personal Preventive Maintenance Service and Repairs for Microbiological Fume Hoods located at Madigan Army Medical Center (MAMC), Joint Base Lewis-McChord (JBLM), Tacoma, WA. These items/supplies/services are for use at MAMC and the proposed contract action is for a base period of one year plus three one year options. All Small Business Contractors shall provide an offer for the following CLINs and option line items. CLIN 0001: The Contractor shall provide a Non-Personal Maintenance Service for the Microbiological Fume Hoods located at Madigan Army Medical Center (MAMC), Joint Base Lewis-McChord (JBLM), Tacoma, WA. The Contractor shall provide fully qualified technicians to perform necessary maintenance service and repairs to the Microbiological Fume Hoods to ensure that optimum operation and efficiency of the equipment is maintained, and that proper functioning of the equipment. The Contractor shall provide unlimited on-call maintenance service. The Contractor shall provide any technical manuals, special tools, supplies, parts and test equipment that is required for the servicing of the Microbiological Fume Hoods. This includes Two (2) Periodic PM services shall be performed for each contract period. PM services shall include scheduled maintenance, inspections, replacement of defective and worn parts, required testing service, and Two (2) periodic Calibration Verification Check (CVC) shall be performed during each period of performance. All labor, travel, equipment, and parts to perform maintenance service and repairs (preventive or corrective) shall be offered at the vendors discounted government rate, including consumables, software updates, etc. Period of Performance: 19 April 2013 through 18 April 2014 ( Subject to the Availability of FY13 Funding (IAW FAR 52.232-18)). CLIN 0002: Option Period One: Description same as CLIN 0001. Period of Performance: 19 April 2014 through 18 April 2015. CLIN 0003: Option Period Two: Description same as CLIN 0001. Sellers MUST enter exactly what they are quoting to include: make, model and description in order for their quote to be considered. Buyer will evaluate quote mark equal items quote mark on the basis of information furnished by the Seller and identified in the quote. Technical Factors are more important than Price. Price must be determined to be fair and reasonable. A. Contractor must provide proof of technical certification. B. Microbiological Fume Hoods must be maintained to conform to the Original Equipment Manufacturer (OEM) standard specifications. C. Microbiological Fume Hoods must be tested semi-annually on-site As Installed (AI) open and must be in accordance with Occupational Safety and Health Administration (OSHA), American Industrial Hygiene Association (AIHA), American National Standard s Institute (ANSI), Scientific Equipment and Furniture Association (SEFA) and American Society of Heating, Refrigerating and Air Conditioning Engineers Inc (ASHRAE). D. Microbiological Fume Hoods Primary Test must include inflow velocity profiling, airflow smoke test, qualitative analysis of air capture and a written evaluation report on each Fume Hood.
